摘要 目的:探讨不同扫描方式对超高龄患者肺部CT图像质量的影响。方法:选取行肺部检查的超高龄患者(≥75岁)100例,采用不同扫描方向和呼吸控制方式随机分为4组,每组25例,A组采用鼻式呼吸行足向头侧扫描,B组采用鼻式呼吸行头向足侧扫描,C组采用呼吸控制行头向足侧扫描,D组采用呼吸控制行足向头侧扫描。将采集的图像按不同的肺部分区进行质量评分和伪影分级统计。结果:A组图像质量评分最高,B组评分最低,4组中、下肺区的图像质量评分和总评分的差异均具有统计学意义(均P<0.05)。组间两两比较,A组与B组、B组与C组、B组与D组的中、下肺区图像质量评分和总评分比较,差异均有统计学意义(均P<0.05)。A组重度伪影发生频率最低,B组最高,A组与B组的无伪影和重度伪影发生频率差异均具有统计学意义(均P<0.05),A组与C、D组的重度伪影发生频率差异均有统计学意义(均P<0.05)。结论:采用鼻式呼吸行足向头侧扫描可减少超高龄患者肺部CT检查的呼吸伪影,提高图像质量,值得临床推广应用。 Objective:To investigate the impact of different scanning methods on the image quality of lung CT in super-elderly patients.Methods:Based on different scanning directions and respiratory control modes,a total of 100 super-elderly patients(≥75 years old)were randomly divided into four groups:Group A,scanning from foot side to head side with nasal respiration;Group B,scanning from head side to foot side with nasal respiration;Group C,scanning from head side to foot side with respiration control;Group D,scanning from foot side to head side with respiration control,with 25 patients in each group.The quality scoring and artifact classification statistics were performed in different lung segments.Results:Group A had the highest quality score and Group B had the lowest score.There were significant differences in image quality scores and total scores in the middle and lower lung areas among the four groups.The pairwise comparison between groups showed there were significant differences in the image quality scores and total scores between Group A and Group B,Group B and Group C,Group B and Group D in the middle and lower lung areas(all P<0.05).In the statistical comparison of artifact classification,Group A had the lowest frequency of severe artifacts,and Group B had the highest.There were significant differences of the frequency of no artifacts and severe artifacts between Group A and B(both P<0.05).There were significant differences in the frequency of severe artifacts between Group A and C,Group A and D(both P<0.05).Conclusion:The scan method from foot side to head side with nasal respiration can reduce respiratory artifacts,improve the image quality in lung CT in super-elderly patients,and is worthy of clinical promotion.
