感染防控路径管理及常态化培训在住院医师群体中的应用与成效

Application and Effectiveness of Epidemic Control Pathway for Resident Standardized Training in Public Health Emergency

目的引入路径管理理念,探索构建信息化住院医师规范化培训和感染防控双联管理模式的价值与成效。方法实施信息化手段辅助的住院医师感染防控路径管理、健康随访、轮科、培训、心理评估和干预;将感染防控课程纳入平台公共课程,防护技能融入住院医师规范化培训(standardized resident training,以下简称“住培”)过程考核。结果实施感染防控路径管理住院医师337人,培训人次334人,考核337人;初期感染防控培训显示,传染病个人防护类题目得分率最低。将感染防控纳入住院医师常态化培训,在后疫情时期感染防控技能考核显示,所有住培专业基地合格率均达100%,与初期培训考核相比有了明显提高;其中放射、超声、检验专业考核平均分增幅显著;防护技能操作成绩中位数分别为87.36、88.9和91.09,个人防护技能成绩明显提高,且连续分析三个月考核成绩提示个人防护技能掌握程度无明显衰减。结论“住院医师规范化培训感染防控路径”指导的住培联合感染防控双联管理模式,对住培科学化、人性化管理,优化临床实践培训、提高住院医师应对突发重大公共卫生事件综合能力等方面具有积极意义。

Objective Explore the two stages of major public health emergencies and normalization of epidemic prevention and control,introduce path management concepts,and build the value and effectiveness of a dual management model for standardized training of informatized residents and infection prevention and control.Methods During the outbreak of the epidemic,implement remote management,health follow-up,rotation,training,psychological evaluation and intervention of residents according to the path;in the general situation of epidemic prevention and control,the infection prevention and control courses are included in the platform public courses,and the protection skills are integrated into the“residential training”Process assessment.Results During the outbreak of the epidemic,337 resident physicians were managed online,334 trained,and 337 assessed;the score rate for personal protection of infectious diseases was the lowest.During the period of normalization of epidemic prevention and control,the theoretical assessment of infection prevention and control showed that the qualification rate of all professional bases reached 100%,which was significantly higher than that of emergency training assessment.Among them,the average score of radiology,ultrasound,and inspection professional assessments increased significantly;protection skills operation results The medians were 87.36,88.9,and 91.09,respectively.The personal protection skills scores improved significantly,and the three-month continuous analysis of the assessment results indicated that the mastery of personal protection skills did not significantly decrease.Conclusion In the context of major public health incidents,the“residential training”combined infection prevention and control dual management model guided by the“standardized training path for resident physicians”will improve the scientific and humanized management of resident physicians and optimize clinical practice training Such aspects are of positive significance.
