直接前方入路全髋关节置换术治疗强直性髋关节炎患者的效果

Effects of total hip replacement with direct anterior approach in treatment of patients with ankylosing arthritis of the hip

目的:观察直接前方入路全髋关节置换术治疗强直性髋关节炎患者的效果。方法:选取80例强直性髋关节炎患者为研究对象,按随机数字表法将其分为对照组和研究组各40例。对照组采用后外侧入路全髋关节置换术治疗,研究组采用直接前方入路全髋关节置换术治疗,比较两组手术时间、术中出血量、手术前后髋关节功能评分和并发症发生率。结果:研究组术中出血量少于对照组,差异有统计学意义(P<0.05);术后3个月,研究组Harris评分高于对照组,差异有统计学意义(P<0.05);研究组并发症发生率为5.0%(2/40),低于对照组的32.5%(13/40),差异有统计学意义(P<0.05)。结论:直接前方入路全髋关节置换术治疗强直性髋关节炎患者可减少术中出血量,提高髋关节功能评分,降低并发症发生率,其效果优于后外侧入路全髋关节置换术治疗。

Objective:To observe effects of total hip replacement with direct anterior approach in treatment of patients with ankylosing arthritis of the hip.Methods:80 patients with ankylosing arthritis of the hip were selected as the research objects,and were divided into control group and study group according to the random number table method,40 cases in each group.The control group was treated with total hip arthroplasty through posterolateral approach,while the study group was treated with total hip arthroplasty through the direct anterior approach.The operation time,intraoperative blood loss,hip function scores before and after the surgery,and the incidence of complications were compared between the two groups.Results:The amount of intraoperative blood loss in the study group was less than that in the control group,and the difference was statistically significant(P<0.05).Three months after the surgery,the Harris score of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Further,the complication rate in the study group was 5.0%(2/40),which was lower than 32.5%(13/40)in the control group,and the difference was statistically significant(P<0.05).Conclusions:The total hip arthroplasty through the direct anterior approach in the treatment of the patients with ankylosing arthritis of the hip can reduce the intraoperative blood loss,improve the hip function scores,and reduce the incidence of complications.Moreover,it is superior to the total hip arthroplasty through the posterolateral approach.
