老年衰弱综合征影响因素分析及与体位性低血压的关系研究

Influencing factors of frailty syndrome and its correlation with orthostatic hypotension in the elderly

目的探讨分析老年衰弱综合征影响因素及与体位性低血压(OH)的关系。方法选取2017年8月至2019年11月浙江医院住院患者420例,根据Fried标准分为衰弱组102例,非衰弱组318例,对两组患者进行日常生活能力评估、睡眠评估及OH测定,比较分析两组衰弱相关数据及临床特征。结果评估为衰弱综合征102例(24.29%),非衰弱综合征318例(75.71%)。两组间性别、年龄、BMI、是否独居、多重共存疾病方面比较差异均无统计学意义(均P>0.05),而有无跌倒史、巴氏指数量表、工具性日常生活能力量表、匹兹堡睡眠质量指数评分、有无OH、多重用药比较差异均有统计学意义(均P<0.05)。结论老年住院人群中衰弱综合征发生率高,跌倒、睡眠、日常生活能力、多重用药为衰弱综合征影响因素,OH与衰弱综合征密切相关。

Objective To investigate the influencing factors of frailty syndrome in elderly people and its relationship with orthostatic hypotension(OH).Methods A total of 420 elderly patients admitted in Zhejiang Hospital from August 2017 to November 2019 were enrolled in the study.There were 102 patients with frailty syndrome(frailty group)and 318 patients without frailty syndrome(non-frailty group)according to the Fried Frailty Scale.Comprehensive geriatric assessment were conducted and the orthostatic blood pressure were measured in supine position in the two groups.The comprehensive assessment data and the incidence of orthostatic hypotension in the two groups were statistically analyzed,and the quantitative data of the two groups were compared using t test.Results Among 420 elderly patients,the incidence of frailty syndrome was 24.29%(102/420).There were 82 cases(19.52%)in the OH group,270 cases(64.29%)living alone,86 cases(20.48%)with a history of falls,222 cases with comorbidities(52.86%),198 cases(47.14%)with multiple medications.There were no significant differences in age,gender,BMI,living alone,and comorbidities between the two groups(all P>0.05).The difference in history of falls,BI,IADL,PSQI scores,OH and multiple medication were statistically significant(P<0.05)between two groups.Conclusion The risk of falling,sleep disorders,ability of daily living and multiple medication are the influential factors of frailty syndrome in the elderly,and orthostatic hypotension is closely related to frailty.
