血细胞比值与狼疮性肾炎患儿疾病活动性指标的关系

Relationship Between Different Blood Cell Ratio and Disease Activity Index in Children with Lupus Nephritis

目的探讨血细胞比值与狼疮性肾炎(LN)患儿疾病活动性指标的关系。方法回顾性分析2012年1月至2019年12月于郑州大学第一附属医院住院初诊的49例LN患儿的临床资料,包括血常规、红细胞沉降率(ESR)、补体C3、系统性红斑狼疮疾病活动性指数(SLEDAI)及肾脏穿刺活检病理等结果。比较疾病活动期与完全缓解期中性粒细胞与淋巴细胞比值(NLR)、嗜碱性粒细胞与淋巴细胞比值(BLR)、嗜酸性粒细胞与淋巴细胞比值(ELR)、单核细胞与淋巴细胞比值(MLR)和血小板与淋巴细胞比值(PLR)的差异。分析活动期NLR、BLR、ELR、MLR、PLR与ESR、补体C3及SLEDAI的关系。结果与完全缓解期比较,活动期NLR、PLR较高(P<0.05);活动期BLR、ELR、MLR与完全缓解期比较,差异无统计学意义(P>0.05)。疾病活动期,NLR与SLEDAI呈正相关(P<0.05),PLR与ESR、SLEDAI呈正相关(P<0.05)。结论NLR、PLR能够反映LN患儿的疾病活动性,可以作为监测LN疾病活动性的炎症指标。

Objective To investigate the relationship between blood cell ratio and disease activity index in children with lupus nephritis(LN).Methods The clinical data of 49 children with initially diagnosed LN admitted to the First Affiliated Hospital of Zhengzhou University from January 2012 to December 2019 were analyzed retrospectively,including routine blood test,erythrocyte sedimentation rate(ESR),complement C3,systemic lupus erythematosus disease activity index(SLEDAI),renal biopsy and so on.Neutrophil to lymphocyte ratio(NLR),basophil to lymphocyte ratio(BLR),eosinophil to lymphocyte ratio(ELR),monocyte to lymphocyte ratio(MLR)and platelet to lymphocyte ratio(PLR)were compared between disease activity period and complete remission period.The relationships between NLR,BLR,ELR,MLR,PLR and ESR,C3 and SLEDAI in active stage were analyzed.Results Compared with complete remission stage,NLR and PLR in active stage were higher(P<0.05).There was no statistical difference in BLR,ELR and MLR between active stage and complete remission stage(P>0.05).In the active stage of disease,NLR was positively correlated with SLEDAI(P<0.05),and PLR was positively correlated with ESR and SLEDAI(P<0.05).Conclusion NLR and PLR could reflect the disease activity of children with LN and could be used as inflammatory indicators to monitor the disease activity of LN.
