Masquelet诱导膜技术与组合式外固定架在距骨感染性骨缺损治疗中的应用 被引量:2

摘要 目的观察Masquelet诱导膜技术联合组合式外固定架固定在距骨感染性骨缺损治疗中的应用效果。方法回顾性分析自2017-01—2020-01诊治的5例距骨感染性骨缺损,一期彻底清创,至骨面出现轻微新鲜渗血,冲洗后置入抗生素骨水泥;待感染控制后,二期手术切开诱导膜,取自体骨完成植骨并进行踝关节融合,最后用组合式外固定架固定。结果5例均获得随访,随访时间平均14.6(8~22)个月。4例切口均一期愈合,1例皮瓣一期成活并修复创面。术后所有患者骨缺损植骨处均获得临床和影像学愈合,愈合时间平均8(2~16)个月。胫距、距下关节融合后拆除外固定架开始负重行走。末次随访时疗效采用踝与后足功能AOFAS评分标准评定:优2例,良2例,可1例。结论Masquelet诱导膜技术联合外固定架固定踝关节融合术是治疗距骨感染性骨缺损的一种有效方法,手术操作简单,早期疗效满意。
