踝足矫形器治疗对脑卒中患者步行功能的影响

Effect of ankle-foot orthosis treatment on walking function of stroke patients

摘要 目的探讨踝足矫形器(AFO)对脑卒中患者步行功能的影响。方法将符合入选标准的40例脑卒中步行功能障碍的患者随机分为观察组(n=20)和对照组(n=20),两组患者均接受常规康复治疗,观察组在此基础上予以AFO治疗,两组患者均使用Senno Gait步态分析系统对其治疗前后进行步态分析。结果经过6个周的治疗,两组患者的步速都较前变快;患侧摆动相的比例都较前变大;患侧支撑相的比例都较前变小;患侧着地仰角都较前变大;患侧着地内翻角都较前变小;患侧最大摆宽都较前变小。两组比较,观察组的各项指标治疗前、后差值较对照组大(P<0.05)。结论脑卒中步行功能障碍患者使用AFO治疗联合常规康复治疗各项指标优于常规康复治疗,因此在常规康复的基础上使用AFO治疗,可以更好、更快地纠正脑卒中导致的步行功能障碍。 Objective To investigate the effect of ankle-foot orthosis(AFO) on the walking function of stroke patients.Methods Forty stroke patients with walking dysfunction who met the inclusion criteria were randomly divided into observation group(n=20) and the control group(n=20),both groups of patients received conventional rehabilitation treatment, and the observation group received AFO treatment on this basis.Both groups of patients used Senno Gait gait analysis system to analyze their gait before and after treatment.Results After 6 weeks of treatment, the pace of the two groups of patients became faster than before treatment;the ratio of the swing phase on the affected side became larger than before treatment;the ratio of support phase on the affected side became smaller than before treatment;the elevation angle of the affected side becomes larger than before treatment;the affected side landed on the ground the turning angle becomes smaller than before treatment;the maximum swing width of the affected side becomes smaller than before treatment.Comparing the two groups, the difference between the observation group’s pace, the affected side swing phase, the affected side support phase, the affected side landing elevation angle, the affected side landing varus angle, and the affected side’s maximum swing width before and after treatment were compared with the control group large(P<0.05).Conclusion In stroke patients with walking dysfunction, the use of AFO therapy combined with conventional rehabilitation therapy is better than conventional rehabilitation.Therefore, the use of AFO therapy on the basis of conventional rehabilitation can better and faster correct the walking function caused by stroke obstacle.
