基于弹性髓内钉与钢板治疗锁骨骨折的安全性及有效性对比

Comparison of the Safety and Effectiveness based on Elastic Intramedullary Nail and Plate in the Treatment of Clavicle Fracture

目的:分析对锁骨骨折使用弹性髓内钉与钢板治疗的效果和安全性。方法:选取于2018年1月—2020年6月间盱眙县人民医院的68例锁骨骨折患者,编号随机抽取,分为两组,其中研究组34例(弹性髓内钉治疗),对照组34例(钢板螺钉治疗),对两组的治疗效果、视觉模拟评分法(VAS)评分、骨折愈合时间、住院时间进行对比,并评价两组患者的并发症、生活质量及满意度情况。结果:治疗后研究组VAS评分(2.88±0.59)比对照组低(P<0.05),住院时间(7.69±1.27)d、骨折愈合时间比对照组短(P<0.05),生活质量比对照组优(P<0.05),关节障碍、切口感染、疼痛、神经受损等并发症发生率8.82%比对照组低(P<0.05),治疗总有效率94.12%比对照组82.35%优(P<0.05),患者满意度94.12%比对照组高(P<0.05)。结论:与钢板螺钉手术治疗相比,锁骨骨折使用弹性髓内钉治疗,效果显著,且疼痛较轻,并发症低,安全性高,缩短骨折愈合时间、患者的生活质量显著提高,能提高患者的满意度。

Objective:To analyze the effect and safety based on elastic intramedullary nail and plate in the treatment of clavicle fracture.Methods:From January 2018 to June 2020,68 patients with clavicle fracture were randomly selected and divided into two groups,including 34 cases in the study group(elastic intramedullary nail treatment)and 34 cases(plate and screw treatment)in the control group.The treatment effect,VAS score,fracture healing time and hospitalization time of the two groups were compared.The complications,quality of life and patient satisfaction of the two groups were evaluated.Results:After treatment,the VAS score of the study group(2.88±0.59)was lower than that of control group(P<0.05).The hospitalization time(7.69±1.27)d and fracture healing time was shorter than those of the control group(P<0.05),the quality of life was better than that of the control group(P<0.05).The incidence of complications such as joint disorders,incision infection,pain,and nerve damage was 8.82%,lower than that of the control group(P<0.05).The total effective rate(94.12%)was better than 82.35%of the control group(P<0.05).Patients satisfaction(94.12%)was higher than that of the control group(P<0.05).Conclusion:Compared with plate and screw surgery,the use of elastic intramedullary nail in the treatment of clavicle fracture has significant effect,less pain,low complications,high safety,shorten the fracture healing time,significantly improve the quality of life of patients,and can improve the satisfaction of patients.
