妊娠期急性脂肪肝的研究进展

妊娠期急性脂肪肝是母体以继发于肝脏脂肪浸润的急性肝衰竭为特征的产科急症,可造成母儿的严重并发症。其发病机制可能与妊娠过程中脂肪酸代谢障碍相关。妊娠期急性脂肪肝与其他的妊娠期肝脏疾病如子痫前期、妊娠期肝内胆汁淤积症等有许多类似之处,早期快速诊断仍存在困难。而延误诊断和治疗会造成母儿方面的严重并发症,所以对于该病早发现和早治疗显得尤为重要。该文对妊娠期急性脂肪肝的流行病学、发病机制、诊断、治疗及预后等方面的研究进展做一综述,希望为改善母儿预后提供帮助。

Acute fatty liver of pregnancy was an obstetric emergency,which was characterized by maternal acute liver failure secondary to fatty infiltration of the liver.It may cause serious complications of mother and child.Its pathogenesis was related to fatty acid metabolism dysfunction during pregnancy.Because there were many similarities between it and other liver diseases during pregnancy,such as preeclampsia,intrahepatic cholestasis of pregnancy,the disease was still difficult to be diagnosed early.However,delayed diagnosis and treatment could cause serious complications for mother and child.Here we review the research progress in the epidemiology,pathogenesis,diagnosis,treatment and prognosis of acute fatty liver of pregnancy.
