隆林多民族地区2型糖尿病多次入院的多因素Logistic回归分析

Multivariate Logistic regression analysis of multiple hospitalizations of patients with type 2 diabetes mellitus in the multi-ethnic area of Longlin

目的探讨隆林多民族地区2型糖尿病(T2DM)患者多次入院的影响因素。方法回顾性研究广西隆林各族自治县人民医院的医院信息系统(hospital information system,HIS)2015年—2020年第一诊断为T2DM的患者住院病案,对比不同组别患者年龄、性别、民族、住院费用、转归等临床资料,采用多因素Logistic回归分析探讨影响多次入院的危险因素。结果共纳入住院患者919例,其中多次入院患者为292例(31.77%)。Logistic回归分析结果显示:男性(OR=1.48,95%CI:1.09~2.02,P=0.01)、>60岁老人(OR=2.49,95%CI:1.42~4.38,P<0.001)、多种并发症(OR=7.38,95%CI:3.31~16.45,P<0.001)是该地区T2DM患者多次入院的独立危险因素,少数民族患者(OR=1.38,95%CI:1.01~1.90,P=0.04)具有多次入院的趋势。结论应针对高龄且具有多种并发症的男性T2DM患者制订血糖调控方案,降低多次入院带来的经济负担及社会负担。

Objective To explore the factors influencing on multiple hospitalizations of patients with type 2 diabetes mellitus(T2DM)in the multi-ethnic area of Longlin.Methods A retrospective study was conducted on the inpatient medical records of the patients first diagnosed with T2DM from 2015 to 2020 in People’s Hospital of Longlin.The medical records were collected from the Hospital Information System(HIS).We compared clinical data such as age,gender,ethnicity,hospitalization cost and outcome of patients in different groups.Multivariate Logistic regression analysis was conducted to explore the risk factors affecting multiple hospitalizations.Results A total of 919 inpatients were included,of which 292 patients(31.77%)were admitted to hospital for multiple times.Logistic regression analysis showed that male(OR=1.48,95%CI:1.09~2.02,P=0.01),being older than 60 years(OR=2.49,95%CI:1.42~4.38,P<0.001)and multiple complications(OR=7.38,95%CI:3.31~16.45,P<0.001)were independent risk factors for multiple hospitalizations of T2DM patients in this area.Minority patients(OR=1.38,95%CI:1.01~1.90,P=0.04)tended to be hospitalized for multiple times.Conclusion Blood glucose regulation programs should be developed for elderly male patients with T2DM and with multiple complications,so as to reduce the economic and social burden caused by multiple hospitalizations.
