酮咯酸氨丁三醇在合并心功能异常患者群体中的应用适应症观察 被引量:1

Observation on Indications of Ketoolac Aminotriol in Patients with Cardiac Dysfunction

摘要 研究酮咯酸氨丁三醇在合并心功能异常患者群体中的应用适应症。回顾性分析从2017年1月~2019年12月,于该院接受手术治疗的患者120例(包括60例下肢骨折手术患者与60例腹腔镜手术患者)为研究组。另取同期接受手术治疗的患者120例(包括60例下肢骨折手术患者与60例腹腔镜手术患者)为对照组。两组患者术后均进行静脉镇痛干预,对照组实施舒芬太尼干预,研究组实施酮咯酸氨丁三醇联合舒芬太尼干预,比较两组镇痛效果以及不良反应发生情况的差异,比较两组不同手术类型患者的手术前后心功能变化水平。两组在术后4,8,24 h VAS评分对比,联合组均显著低于对照组(均P <0.05)。两组不良反应总发生率对比,联合组显著低于对照组(P <0.05)。两组和对照组下肢骨折手术患者在术前、术后1 h时的CO、SV、ACI水平方面对比,差异均不明显(均P> 0.05)。两组腹腔镜手术患者在术前、术后1 h时的CO、SV、ACI水平对比,差异均不明显(均P> 0.05)。酮咯酸氨丁三醇应用于合并心功能异常患者中的镇痛效果明显,安全性较好,且适用于合并心功能异常的下肢骨折手术以及腹腔镜手术患者中。 To study the indications of ketoolac aminotriol in patients with cardiac dysfunction,a retrospective analysis was performed on 120 patients who received surgical treatment in the author’s hospital from January 2016 to January 2019,and they were recorded as study group( including 60 patients with lower limb fracture surgery and 60 patients with laparoscopic surgery). In addition,120 patients who received surgical treatment in the author’s hospital during the same time were taken as the control group( including 60 patients with lower limb fracture surgery and 60 patients with laparoscopic surgery). Both groups received intravenous analgesia intervention,the control group received sufentanil intervention,and the combination group received ketoclolac aminotriol combined with sufentanil intervention. The differences in analgesic effect and adverse reactions between the two groups were compared. In addition,the changes in cardiac function before and after surgery in patients of different surgical types were compared. VAS scores 4,8,24 h postoperatively of the combined group were significantly lower than those of the control group( P < 0. 05). The total incidence of adverse reactions was significantly lower in the combined group than that in the control group( P < 0. 05). The CO,SV and ACI levels of lower limb fracture patients in the combined group and the control group before and 1 hour after surgery were not significantly different( all P > 0. 05). The CO,SV and ACI levels of the combined group and the control group before and 1 hour after laparoscopic surgery were not significantly different( all P > 0. 05). Ketoolac aminotriol had obvious analgesic effect and good safety in patients with abnormal heart function,and was suitable for lower limb fracture surgery and laparoscopic surgery in patients with abnormal heart function.
