体外受精/卵细胞浆内单精子注射-胚胎移植后早期流产影响因素分析

Influencing factors of early abortion following in vitro fertilization/intracytoplasmic sperm injection-embryo transfer

目的分析体外受精/卵细胞浆内单精子注射-胚胎移植(IVF/ICSI-ET)后早期流产影响因素。方法收集IVF/ICSI-ET后妊娠者820例,分为早期流产组(105例)和继续妊娠组(715例),比较两组临床和实验室检查指标。结果IVF/ICSI-ET后早期流产率为12.8%。早期流产组的年龄、启动日雌二醇水平及身体质量指数(BMI)高于继续妊娠组(P<0.01或0.05)。随着年龄增大、BMI增加,流产率也不断升高(P<0.01或0.05)。早期流产组胚胎绒毛染色体检测异常率为59.7%(37/62)。结论IVF/ICSI-ET早期流产主要因素为胚胎染色体异常,而高龄和肥胖患者也会增加早期流产风险。

Objective To analyze the influencing factors of early abortion after in vitro fertilization/intracytoplamsic sperm injection-embryo transfer(IVF/ICSI-ET).Methods A total of 820 pregnant women following IVF/ICSI-ET were enrolled,including 105 subjects with early abortion group and 715 with continued pregnancy.The clinical and laboratory parameters were compared between 2 groups.Results The early abortion rate was 12.8%in 820 pregnant women following IVF/ICSI-ET.The age,estradiol level at initial day and body mass index(BMI)were higher in early miscarriage group than in continued pregnancy group(P<0.01 or 0.05).The miscarriage rate was elevated with the increase of age and BMI(P<0.01 or 0.05).The abnormal rate of chromosomes in embryonic villi was 59.7%(37/62)in early abortion group.Conclusion The embryo chromosomal abnormality is the main cause of early abortion following IVF/ICSI-ET,while the risk of early abortion is also high in the older and obese women.
