老年高血压患者动态血压监测结果与MMSE评分相关性分析

Correlation Analysis of Ambulatory Blood Pressure Monitoring and MMSE Score in Elderly Patients with Hypertension

目的探讨老年高血压患者动态血压监测结果与简易智能精神状态检查量表(MMSE)评分相关性。方法选择2017年4月至2020年5月我院收治的90例老年高血压患者为研究对象,按照患者认知功能情况分为两组,认知功能障碍组42例,认知功能正常组48例。对比两组动态血压监测指标差异,分析认知功能障碍老年高血压患者的动态血压监测指标与MMSE相关性。结果认知功能障碍组24 h平均收缩压、24 h收缩压变异性值高于认知功能正常组,24 h最小舒张压值低于认知功能正常组,差异有统计学意义(P<0.05);两组24 h最小收缩压、夜间平均收缩压、白天平均收缩压、24 h平均舒张压、清晨收缩压、清晨舒张压、夜间平均舒张压、白天平均舒张压、24 h舒张压变异性值相比,差异无统计学意义(P>0.05);24 h平均收缩压、24 h收缩压变异性与MMSE评分负相关(P<0.05),24 h最小舒张压与MMSE评分呈正相关(P<0.05);24 h最小收缩压、夜间平均收缩压、白天平均收缩压、24 h平均舒张压、清晨收缩压、清晨舒张压、夜间平均舒张压、白天平均舒张压、24 h舒张压变异性与MMSE评分无相关性(P>0.05)。结论老年高血压患者动态血压监测结果中24 h平均收缩压、24 h收缩压变异性、24h最小舒张压与MMSE评分密切相关,有效改善上述指标可能会延缓患者认知功能减退。

Objective To investigate the correlation between the results of ambulatory blood pressure monitoring in elderly patients with hypertension and the score of simple intelligent mental state examination scale(MMSE).Methods From April 2017 to May 2020,90 elderly patients with hypertension in our hospital were selected as the research subjects,which were divided into two groups according to the cognitive function of the patients,42 cases of cognitive dysfunction group and48 cases of normal cognitive function group.Compare the difference of ambulatory blood pressure monitoring index between the two groups,and analyze the correlation between ambulatory blood pressure monitoring index and MMSE in elderly patients with cognitive dysfunction.Results 24 h mean systolic blood pressure,24 h systolic blood pressure in cognitive impairment grouph min systolic blood pressure,nocturnal mean systolic blood pressure,daytime mean systolic blood pressure,24 h mean diastolic blood pressure,morning systolic blood pressure,early morning diastolic blood pressure,nocturnal mean diastolic blood pressure,daytime mean diastolic blood pressure,24 h diastolic blood pressure variability were not correlated with MMSE score(P<0.05).Conclusion 24 h mean systolic blood pressure,24 h systolic blood pressure variability,24 minimum diastolic blood pressure and MMSE score are closely related in the monitoring results of ambulatory blood pressure in elderly patients with hypertension.Effective improvement of these indicators may delay cognitive decline in patients.
