单一髂腹股沟入路联合顺行拉力螺钉治疗髋臼骨折 被引量:2

摘要 目的探讨单一髂腹股沟入路联合顺行拉力螺钉治疗髋臼骨折的疗效。方法采用单一髂腹股沟入路联合顺行拉力螺钉治疗8例髋臼骨折患者。记录复位质量、愈合时间、髋关节功能及并发症发生情况。结果患者均获得随访,时间12~18(14.5±3.2)个月。术后切口周围软组织感染1例,经引流及抗感染治疗后伤口愈合。术后2例出现患侧大腿前外侧麻木,术后3个月逐渐消失。术后按Matta评分标准评价髋臼骨折复位质量:优6例,良1例,差1例。骨折均愈合,时间10~16(12.5±3.6)周。末次随访时采用改良Merle d’Aubigné&Postel评分标准评定患髋功能:优7例,良1例。结论采用单一髂腹股沟入路联合顺行拉力螺钉治疗髋臼骨折,复位好,固定有效,创伤小,效果好。 Objective To investigate the effect of single ilioinguinal approach combined with antegrade lag screw for treatment of acetabular fracture. Methods Eight cases of acetabular fractures were treated by single ilioinguinal approach combined with antegrade lag screw. The postoperative fracture reduction quality,fracture healing time,hip joint function and complications were recorded. Results All patients were followed up for 12 ~ 18( 14. 5 ± 3. 2) months.One case of soft tissue infection occurred around the incision,and healed after drainage and anti-infection treatment.There were 2 cases with anterolateral numbness of the affected thigh,and gradually disappeared at 3 months postoperation. The reduction quality of acetabular fracture after operation was evaluated according to Matta scoring standard:the result was excellent in 6 cases,good in 1,poor 1. All fractures healed,the healing time was 10 ~ 16( 12. 5 ± 3. 6)weeks. At the last follow-up,the modified Merle d’Aubigné and Postel score criteria were used to evaluate the hip function: excellent in 7 cases,good in 1. Conclusions Single ilioinguinal approach combined with antegrade lag screw internal fixation for acetabular fractures has the advantages of good reduction,effective fixation,less trauma and good effect.
