摘要
目的实时三维超声心动图(RT-3DE)观察心肌梗死患者左心室收缩功能。方法方便选取该院2017年10月—2018年10月期间收入的左心室心肌梗死患者60例进行研究,由于梗死位置不同分为4组,其中下壁组13例、后壁组15例、前壁及前间壁组17例、侧壁组15例,利用二维超声心动图(2DE)Simpson法、实时三维超声心动图(RT-3DE)全容积法、核磁共振成像(CMRI)测量患者左室舒张末容积(LVEDV)、每搏输出量(SV)、左心室射血分数(LVEF)。结果2DE与CMRI监测4组患者LVEDV、SV,差异有统计学意义(P<0.05),RT-3DE与CMRI监测4组患者LVEDV、SV、LVEF差异无统计学意义(P>0.05);RT-3DE、CMRI、LVEDV、SV、LVEF存在相关性(下壁组r=0.86,r=0.83,r=0.91;后壁组r=0.85,r=0.91,r=0.93;前壁及前间壁组r=0.86,r=0.87,r=0.92;侧壁组r=0.84,r=0.86,r=0.93,P<0.05)。结论RT-3DE能够很好地评价心肌梗死患者左心室收缩功能,对患者诊断、判断预后具有重要意义,对患者心功能的评价更加准确。
Objective Real-time three-dimensional echocardiography(RT-3DE)observes left ventricular systolic function in patients with myocardial infarction.Methods A total of 60 patients with left ventricular myocardial infarction received in the hospital from October 2017 to October 2018 were conveniently selected.They were divided into four groups due to different infarct locations,including 13 cases in the inferior wall group,15 cases in the posterior wall group,and anterior wall of 17 cases in the anterior septum group and 15 cases in the lateral wall group.The left ventricular end diastolic volume(LVEDV),volume per stroke(SV)and Left ventricular ejection volume(LVEF)of the patients were measured by two-dimensional echocardiography(2DE)Simpson method,real-time three-dimensional echocardiography(RT-3DE)full volume method,and magnetic resonance imaging(CMRI).Results The difference in LVEDV and SV between the 4 groups monitored by 2DE and CMRI was statistically significant(P<0.05),and the difference in LVEDV,SV,LVEF monitored by RT-3DE and CMRI between 4 groups of patients was not statistically significant(P>0.05).RT-3DE full volume method,LVEDV,SV,and LVEF under CMRI are correlated(inferior wall group r=0.86,r=0.83,r=0.91;posterior wall group r=0.85,r=0.91,r=0.93;anterior wall group And anterior partition group r=0.86,r=0.87,r=0.92;side wall group r=0.84,r=0.86,r=0.93,P<0.05).Conclusion RT-3DE can evaluate the systolic function of the left ventricle in patients with myocardial infarction,which is of great significance for the diagnosis and prognosis of patients,and the evaluation of the cardiac function of patients is more accurate.
