3D打印技术在高血压脑出血软通道穿刺引流中的应用 被引量:5

摘要 目的探讨3D打印技术在高血压脑出血软通道穿刺引流中的应用效果。方法选取2019年3月至2021年2月本院收治的60例高血压脑出血患者,随机分为实验组和对照组,每组30例。两组均予以微创软通道穿刺引流术,实验组利用数字化设计和3D打印技术研制个体化导板。比较两组围手术期情况、血肿清除情况、术后并发症发生率及预后情况。结果实验组手术时间、住院时间均短于对照组,术中出血量、残余血肿量均少于对照组,差异有统计学意义(P<0.05);术后1、3 d,实验组血肿清除量均高于对照组,差异有统计学意义(P<0.05);术后7 d,两组血肿清除量比较差异无统计学意义;术后,实验组颅内再出血、颅内感染、肺部感染发生率均明显低于对照组,差异有统计学意义(P<0.05);术后3个月,实验组预后良好率高于对照组,差异有统计学意义(P<0.05)。结论3D打印技术在高血压脑出血软通道穿刺引流中的应用效果显著,3D打印技术个体化制造导板应用于软通道穿刺引流,可有效防止盲穿对脑组织的大面积损伤,且操作简便,可广泛在基层医院进行推广和应用,有助于指导临床经验不足的医生学习穿刺技术。 Objective To explore the application effect of 3D printing technology in soft channel puncture and drainage of hypertensive cerebral hemorrhage.Methods 60 patients with hypertensive cerebral hemorrhage admitted to our hospital from March 2019 to February 2021 were random-ly divided into experimental group and control group,with 30 cases in each group.Two groups were given minimally invasive soft channel puncture and drainage,the experimental group used digital design and 3D printing technology to develop personalized guide plate.The perioperative situation,hematoma clearance,postoperative complications and prognosis of the two groups were compared.Results The operation time and hospitalization time were shorter than those in the control group,intraoperative blood loss and residual hematoma volume in the experimental group were less than those in the control group,the difference was statisticallysignificant(P<0.05);at 1 and 3 days after operation,the hematoma clearance in the experi-mental group was higher than that in the control group,the difference was statisticallysignificant(P<0.05);7 d after operation,there was no signifi-cant difference in the hematoma clearance amount between the two groups;after operation,the occurrence of intracranial rebleeding,intracranial in-fection and pulmonary infection in the experimental group were significantly lower than those in the control group;the good prognosis rate of the ex-perimental group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion 3D printing technology in the application of hypertension cerebral hemorrhage soft channel puncture drainage effect is remarkable,using 3D printing individualized manufac-turing guide used in soft channel puncture drainage to prevent blindness in large area of brain tissue damage,and the 3D printing technology is sim-ple,can be widely popularized and applied in basic-level hospitals,help guide clinical inexperienced doctors learn puncture technology.
