上消化道出血患者凝血纤溶系统指标变化及护理措施

Changes of Coagulation and Fibrinolysis System Indexes in Patients with Upper Gastrointestinal Bleeding and Nursing Measures

目的探讨上消化道出血患者凝血纤溶系统指标变化及护理措施。方法以80例上消化道出血患者为对象,以同期80例健康人为对照组。比较两组凝血、纤溶指标水平。结果观察组活化部分凝血活酶时间(aPTT)、凝血酶原时间(PT)、凝血酶时间(TT)、纤维蛋白原(Fbg)水平高于对照组(P<0.05)。观察组D二聚体(D-D)水平高于对照组,组织型纤溶酶原激活剂(t-PA)、纤溶酶原激活物抑制因子(PAI)水平低于对照组(P<0.05)。再出血组与未出血组aPTT、PT、TT、Fbg水平均降低,未出血组降低程度高于再出血组(P<0.05)。再出血组与未出血组治疗后D-D水平降低t-PA、PAI水平升高,未出血组变化程度高于再出血组(P<0.05)。结论上消化道出血患者凝血、纤溶功能指标均出现明显变化,治疗期间配合护理干预,可有效提升止血效果。

Objective To investigate the changes of coagulation and fibrinolysis system indexes in patients with upper gastrointestinal bleeding and nursing measures.Methods 80 patients with upper gastrointestinal bleeding were selected as the objects,and 80 healthy people were taken as the control group.The coagulation and fibrinolysis indexes were compared between the two groups.ResultsThe levels of activated partial prothrombin time(aPTT),prothrombin time(PT),thrombin time(TT)and fibrinogen(Fbg)in the observation group were higher than those in the control group(P<0.05).The level of D-dimer(D-D)in the observation group was higher than that in the control group,while the levels of tissue type plasminogen activator(t-PA)and plasminogen activator inhibitor(PAI)in the observation group were lower than those in the control group(P<0.05).The levels of aPTT,PT,TT and Fbg in rebleeding group and non bleeding group were decreased,and the degree of reduction in non bleeding group was higher than that in rebleeding group(P<0.05).After treatment,the levels of D-D decreased and the levels of t-PA and PAI increased in rebleeding group and non bleeding group,and the change degree of non bleeding group was significantly higher than that of rebleeding group(P<0.05).Conclusion The coagulation and fibrinolysis function indexes of patients with upper gastrointestinal bleeding have obvious changes.Nursing intervention during the treatment can effectively improve the hemostasis effect.
