妊娠期糖尿病胰岛素治疗的效果及对母婴结局的影响

Effect of Insulin Therapy on Gestational Diabetes and Its Influence on the Outcomes of Mother and Infant

目的探究妊娠期糖尿病胰岛素治疗的效果及对母婴结局的影响。方法以2018年1月—2020年2月妊娠期糖尿病患者100例为研究对象,随机数字表法分为研究组(n=50)和对照组(n=50),其中对照组实施饮食、运动等常规治疗干预,研究组则在对照组的基础上实施胰岛素治疗,分析两组妊娠期糖尿病患者不良产妇结局、围生儿结局、空腹血糖、餐后2 h血糖、糖化血红蛋白水平、低血糖发生等情况。结果不良产妇结局发生率相比,研究组更低,差异有统计学意义(P<0.05);不良围生儿结局发生率相比,研究组也更低,差异有统计学意义(P<0.05);治疗后,空腹血糖、餐后2 h血糖、糖化血红蛋白水平相比,均较治疗前降低,且研究组相较于对照组更低,差异有统计学意义(P<0.05);两组妊娠期糖尿病患者低血糖发生率相比,差异无统计学意义(P>0.05)。结论妊娠期糖尿病治疗中,实施胰岛素干预,能够更好改善其血糖水平,降低母婴风险,且安全性较高。

Objective To explore the effect of insulin therapy for gestational diabetes and its influence on the outcomes of mother and infant.Methods 100 cases of gestational diabetes patients from January 2018 to February 2020 were taken as the research objects.Random number table method was divided into study group(n=50)and control group(n=50).The control group was given diet conventional interventions such as diet,exercise,and other routine treatments.The study group implemented insulin therapy on the basis of the control group.The two groups of gestational diabetes patients were analyzed for adverse maternal outcomes,perinatal outcomes,fasting blood glucose,2 h postprandial blood glucose,glycosylated blood glucose protein level,and hypoglycemia occurrence and so on.Results Compared with the study group,the incidence of adverse maternal outcomes was lower,the difference was statistically significant(P<0.05);compared with the incidence of adverse perinatal outcomes,the study group was also lower,the difference was statistically significant(P<0.05);after treatment,compared with fasting blood glucose,and the blood glucose level of 2 h after meal,the levels of glycosylated hemoglobin were lower than before treatment,and the study group was lower than the control group,the difference was statistically significant(P<0.05);compared with the incidence of hypoglycemia in the two groups of gestational diabetes patients,the difference was not statistically significant(P>0.05).Conclusion In the treatment of gestational diabetes,the implementation of insulin intervention can better improve the blood glucose level,reduce the risk of mother and child,and has high safety,which is of great significance.
