摘要
目的探讨ω-3鱼油脂肪乳对腹腔镜原发性肝癌切除术后患者炎性反应的影响。方法2018年1月至2019年12月我院收治的腹腔镜原发性肝癌切除术患者76例,其中36例患者术后肠外营养支持中添加ω-3鱼油脂肪乳(鱼油组),40例患者术后采用常规肠外营养支持(常规组)。比较两组患者围手术期肝功指标、炎性反应指标变化及手术并发症发生情况。结果两组患者在切肝范围、入肝血流阻断时间、手术时间、术前肝功能指标及炎性反应指标等方面差异无统计学意义(P>0.05);术后第3天两组肝功能指标及炎性指标较术前均明显升高,常规组升高更为显著(P<0.05);术后第7天两组肝功能指标及炎性指标均有所好转,鱼油组恢复更快(P<0.05);鱼油组肝功能衰竭发生率显著低于常规组(P<0.05),两组肝断面感染、肺炎、胆漏等并发症发生率比较差异无统计学意义(P>0.05),但常规组总体并发症发生率显著高于鱼油组(P<0.05)。结论ω-3鱼油脂肪乳可降低腹腔镜原发性肝癌切除患者术后炎症反应,减轻术后肝功能损伤,减少总体并发症发生率。
Objective To explore the effect ofω-3 fish oil emulsion on inflammatory reaction in patients with liver cancer after laparoscopic resection.Methods The clinical data of 76 patients admitted to our hospital with primary liver cancer treated by laparoscopic surgery from January 2018 to December 2019 were retrospectively analyzed.Of the patients,36 patients were selected as fishoil group in whichω-3 fish oil emulsion was added into postoperative parenteral nutrition support.Another 40 patients who received conventional parenteral nutrition support after operation were selected as control group.The changes of liver function index,inflammatory reaction indexes,and the incidence of surgical complications were compared between the two groups.Results There was no significant difference in the scopeofliverresection,the time of hepatic inflow occlusionandoperation time between the two groups(P>0.05).On the third day after operation,the liver function indexes and inflammatory indexes of the two groups were significantly higher than those before the operation,and the increase was more significant in the control group(P<0.05).On the 7 th day after surgery,the liver function indexes and inflammatory indexes of the two groups of patients were improved,but the fish oil group recovered faster(P<0.05).The incidence of liver failure in the fish oil group was significantly lower than that in the control group(P<0.05).There was no significant difference in the incidence of abdominal infection,pulmonary infection and bile leakage between the two groups(P>0.05),but the overall incidence of complications in the control group was significantly higher than that in the fish oil group(P<0.05).Conclusionω-3 fish oil emulsion can reduce the postoperative inflammatory reaction,the liver function damage and the overall incidence of complications.
