1例瓣膜置换术后患者的用药方案分析

Analysis of drug regimen of one patient after valve replacement

心脏瓣膜置换术后的抗凝治疗具有非常重要的临床意义,同时合理的抗凝治疗对于提高房颤患者的远期预后也具有重要的意义。本文记录的案例患者为心脏机械瓣膜置换术后伴房颤,具有更高的血栓栓塞发生风险,因此个体化的抗凝治疗显得尤为重要。本文通过对患者整个药物治疗过程进行监护,结合患者病情变化和临床治疗效果对抗凝药物、抗菌药物和心力衰竭治疗药物进行调整,最终制定适合患者的治疗方案,提高临床药师的实践工作能力,并为慢病患者长期有效、安全的药物治疗提供保障。

Anticoagulant therapy after heart valve replacement is of great clinical significance,and reasonable anticoagulant therapy is also of great significance for improving the long-term prognosis of patients with atrial fibrillation.The case recorded in this paper was a patient with atrial fibrillation after mechanical heart valve replacement,which has a higher risk of thromboembolism.Therefore,individualized anticoagulant therapy is particularly important.This article through the whole process of drug therapy for patients with monitoring,combining with the condition changes and clinical therapeutic effect of anticoagulant drugs,antibacterial and adjust heart failure treatment,eventually making treatment plan for patients,improve the practice ability of clinical pharmacists,and patients long-term effective and safe drug treatment is slow.
