经阴道自然腔道内镜下卵巢囊肿剥除术的临床效果

Clinical effect of oophorocystectomy under transvaginal natural orifice transluminal endoscope

目的探讨经阴道自然腔道内镜手术(vNOTES)治疗卵巢囊肿的临床疗效。方法84例良性卵巢肿瘤患者随机分为vNOTES组和单孔腹腔镜手术(LESS)组,每组42例。比较两组基本临床资料、手术时间、术后实验室指标、肛门排气时间、住院时间和术后24 h VAS疼痛评分。结果两组患者的年龄、BMI、既往盆腹腔手术史、卵巢体积、病理类型、手术时间、住院时间、排气时间、术后白细胞和中性粒细胞计数以及CRP水平差异均无统计学意义(P>0.05),而vNOTES组患者术后24 h VAS疼痛评分低于LESS组(P<0.05)。结论在充分评估病情的基础上,vNOTES在减轻卵巢囊肿剥除术患者疼痛和避免体表瘢痕方面优于LESS。

Objective To observe the clinical efficacy of transvaginal natural orifice transluminal endoscopic surgery(vNOTES)in the treatment of ovarian cysts.Methods Eighty-four patients with benign ovarian tumor were equally randomized into two groups of vNOTES and LESS(laparoendoscopic single-site surgery).The basic clinical data,operation time,postoperative laboratory index,exhaust time,hospital stay and VAS pain score in the 24th hour after operation were compared between the two groups.Results There were no statistical differences in the age,BMI,history of pelvic and abdominal operation,ovarian size,pathological type,operation time,hospital stay,exhaust time,amounts of postoperative WBC and neutrophil and level of CRP between groups of vNOTES and LESS(P>0.05).The VAS pain score in the 24th hour after operation in group vNOTES was less than that in group LESS(P<0.05).Conclusion Based on fully evaluating the patient’s condition,vNOTES is better than LESS in reducing pain and avoiding scar on the body surface in the patients undergoing oophorocystectomy.
