免疫球蛋白和肝功能指标检测在肝硬化慢性肝衰竭患者预后评估中的作用

Role of detection of immunoglobulin and liver function indexes in prognosis evaluation of patients with cirrhosis and chronic liver failure

目的探讨免疫球蛋白和肝功能指标检测在肝硬化慢性肝衰竭患者预后评估中的作用。方法回顾性分析90例肝硬化慢性肝衰竭患者的免疫球蛋白和肝功能指标水平,统计1年内患者的生存情况。采用Logistic回归模型和受试者工作曲线(ROC)分析各指标对肝硬化慢性肝衰竭患者预后的评估价值。结果死亡组的IgA、IgG、IgM、AST、ALT、TBIL水平明显高于生存组(P<0.05)。Logistic回归分析结果表明,IgA、IgG、IgM、AST、ALT、TBIL均是肝硬化肝衰竭患者死亡的独立危险因素(P<0.05)。AST和TBIL预测肝硬化慢性肝衰竭患者死亡的约登指数较高,分别为91.17%和82.34%。结论免疫球蛋白和肝功能指标检测在肝硬化慢性肝衰竭患者不良预后评估中具有一定的应用价值。

Objective To investigate role of detection of immunoglobulin and liver function indexes in prognosis evaluation of patients with cirrhosis and chronic liver failure.Methods The levels of immunoglobulin and liver function indexes in 90 patients with cirrhosis and chronic liver failure were analyzed retrospectively,and the survival of the patients within one year was counted.Logistic regression model and receiver operating curve(ROC)were used to analyze the evaluation value of prognosis of each index in patients with cirrhosis and chronic liver failure.Results The levels of IgA,IgG,IgM,AST,ALT and TBIL in the death group were significantly higher than those in the survival group(P<0.05).Logistic regression analysis results showed that IgA,IgG,IgM,AST,ALT and TBIL were independent risk factors of death in patients with cirrhosis and chronic liver failure(P<0.05).The Youden index of AST and TBIL in the evaluation of death of cirrhosis and chronic liver failure patents were higher,with 91.17%and 82.34%respectively.Conclusion Immunoglobulin and liver function index have a certain application value in the evaluation of poor prognosis in patients with cirrhosis and chronic liver failure.
