呼出气一氧化氮与血清总免疫球蛋白E在哮喘-慢性阻塞性肺疾病重叠综合征诊断中的应用

摘要 目的探讨呼出气一氧化氮(FeNO)与血清总免疫球蛋白E(T-IgE)在哮喘-慢性阻塞性肺疾病重叠综合征(ACOS)诊断中的应用。方法将2017年1月至2019年3月福建三明市第二医院收治的70例支气管哮喘患者纳入A组;将同期收治的70例慢性阻塞性肺疾病(COPD)患者纳入B组;将同期收治的70例ACOS患者纳入C组,分析三组的基线资料、实验室指标(FeNO、T-IgE)及FeNO联合T-IgE鉴别诊断ACOS的价值。结果A组FeNO、血清T-IgE水平最高,其次为C组,B组最低,三组间比较,差异有统计学意义(P<0.05)。绘制受试者工作特征曲线结果显示,FeNO、血清T-IgE单独及联合鉴别诊断ACOS的曲线下面积均>0.7,具有一定预测价值,且联合检测诊断价值最高。结论FeNO及T-IgE在ACOS中呈异常表达,FeNO联合T-IgE可有效鉴别ACOS,具有较高诊断效能。 Objective To explore the application of exhaled nitric oxide(FeNO)and serum total immunoglobulin E(T-IgE)in the diagnosis of asthma chronic obstructive pulmonary disease overlap syndrome(ACOS).Methods From January 2017 to March 2019,70 patients with bronchial asthma in Sanming Second Hospital of Fujian Province were included in group A.70 patients with chronic obstructive pulmonary disease(COPD)were included in group B.70 patients with ACOS in the same period were included in group C.The baseline data,laboratory indexes(FeNO,T-IgE)and the value of FeNO combined with T-IgE in the differential diagnosis of ACOS were analyzed.Results the levels of FeNO and serum T-IgE in group A were the highest,followed by group C,and the lowest in group B.the differences among the three groups were statistically significant(P<0.05).The area under the curve of FeNO and serum T-IgE in the differential diagnosis of ACOS was more than 0.7,which had a certain predictive value,and the combined detection had the highest diagnostic value.Conclusion FeNO and T-IgE are abnormally expressed in ACOS.FeNO combined with T-IgE can effectively differentiate ACOS with high diagnostic efficiency.
