股骨转子间骨折患者应用股骨近端防旋髓内钉内固定的隐性失血问题

Hidden Blood Loss of Patients with Femoral Intertrochanteric Fractures Using Proximal Femoral Nail Antirotation(PFNA)Internal Fixation

目的研究股骨转子间骨折患者应用股骨近端防旋髓内钉(PFNA)内固定的隐性失血问题。方法选取2018年10月至2020年5月福建省老年医院收治的88例股骨转子间骨折患者作为研究对象,根据治疗方式的不同将患者分为对照组和试验组。对照组32例,采用动力髋螺钉(DHS)内固定手术治疗,试验组56例,采用PFNA内固定手术治疗。比较两组手术时间、骨折愈合时间、术后隐性失血量。结果试验组手术时间、骨折愈合时间、术后隐性失血量均优于对照组,差异有统计学意义(P<0.05)。结论股骨转子间骨折患者应用DHS或PFNA内固定手术治疗方式均会发生隐性失血问题,PFNA相较于DHS的效果更确切,且隐性失血量较少,是股骨转子间骨折患者优选治疗方式。

Objective To study the hidden blood loss of patients with femoral intertrochanteric fractures using proximal femoral nail antirotation(PFNA)internal fixation.Methods From October 2018 to May 2020,88 patients with femoral intertrochanteric fractures who were treated in Fujian Provincial Geriatric Hospital were selected as the research object.According to different treatment methods,the patients were divided into the control group(32 cases)and the experimental group(56 cases).The control group received dynamic hip screw(DHS)internal fixation surgery treatment,and the experimental group received the PFNA internal fixation surgical treatment.The operation time,fracture healing time and postoperative hidden blood loss between two groups were compared.Results The operation time,fracture healing time and postoperative hidden blood loss of the experimental group were better than those of the control group,and the differences were statistically significant(P<0.05).Conclusions Patients with femoral intertrochanteric fractures who are treated with DHS or PFNA internal fixation will have hidden blood loss.PFNA is more effective than DHS and is the preferred treatment for patients with femoral intertrochanteric fractures.
