参苓白术散加味对亚急性湿疹患者免疫功能及炎性因子水平的影响

Effect of Modified Shenling Baizhu Powder on Immune Function and Inflammatory Factors in Patients with Subacute Eczema

目的探讨参苓白术散加味对亚急性湿疹患者免疫功能及炎性因子水平的影响。方法2018年9月—2020年5月,从治疗的亚急性湿疹患者中选取76例,根据随机数字表法对其进行分组处理,其中对照组有38例,试验组有38例。对照组患者给予盐酸左西替利嗪片治疗,试验组在对照组的基础上加用参苓白术散加味治疗。14 d为一个疗程,2组均治疗3个疗程。对比2组患者治疗后的总有效率、治疗前后免疫功能指标水平及血清TNF-α、IL-2、IL-4水平变化。结果治疗后,试验组治疗总有效率为94.74%,高于对照组患者的73.68%;治疗后,2组患者外周血CD4^(+)及CD4^(+)/CD8^(+)水平相比于治疗前,均升高,且试验组相比于对照组,呈升高趋势;治疗后2组患者外周血CD8^(+)水平及血清TNF-α、IL-2、IL-4相比于治疗前,均降低,且试验组相比于对照组,呈降低趋势(均P<0.05)。结论参苓白术散加味可提高亚急性湿疹患者的治疗总有效率,改善机体免疫功能,同时可降低患者血清TNF-α、IL-2、IL-4水平,进而抑制机体炎性反应。

Objective To study the effects of modified Shenling Baizhu powder on immune function and inflammatory factors in patients with subacute eczema.Methods From September 2018 to May 2020,76 patients with subacute eczema treated in our hospital were selected.According to the random number table method,they were divided into groups,and there were 38 cases in the control group and 38 cases in the treatment group.Patients in the control group were treated with levocetirizine hydrochloride tablets,and patients in the treatment group were treated with modified Shenling Baizhu powder on the basis of the control group.A course of treatment was 14 d,and both groups were treated for 3 courses.The total effective rate after treatment were compared,the changes of immune function indexes and the serum TNF-α,IL-2 and IL-4 levels before and after treatment were compared between the two groups of patients.Results After treatment,the total effective rate of the treatment group was 94.74%which was higher than 73.68%of the control group.After treatment,the levels of CD4^(+) and CD4^(+)/CD8^(+) in peripheral blood of the two groups were higher than those before treatment,compared with the control group,the treatment group showed an increasing trend.After treatment,the levels of CD8^(+) in peripheral blood and serum TNF-α,IL-2 and IL-4 in the two groups were lower than those before treatment,and the treatment group showed a downward trend(all P<0.05).Conclusion Modified Shenling Baizhu powder can improve the total effective rate of patients with subacute eczema,improve the immune function,and reduce the serum levels of TNF-α,IL-2 and IL-4,thereby inhibiting the body’s inflammatory reaction.
