补肾壮骨方对高龄股骨粗隆间骨折术后关节功能及骨代谢的影响

Effect of Bushen Zhuanggu Recipe on Joint Function and Bone Metabolism in Elderly Patients with Intertrochanteric Fracture after Operation

目的探讨补肾壮骨方对高龄股骨粗隆间骨折术后患者关节功能及骨代谢的影响。方法回顾性分析杭州市余杭区中医院骨伤科2017年1月—2019年6月收治的高龄股骨粗隆间骨折内固定术后患者66例,以治疗方法分为2组,各33例。对照组采用钙尔奇D治疗,试验组则联合补肾壮骨汤治疗,3周为一个疗程,2组均治疗3个疗程。比较2组患者术后3个月临床疗效,术前、术后30 d的髋关节功能和VAS评分;术前、术后90 d血清P1NP、β-CTX、OCN水平变化。结果治疗3个月后,试验组的总有效率为84.85%,高于对照组66.67%;与术前相比,术后30 d 2组患者的VAS评分降低,且试验组低于对照组,而Harris评分升高,且试验组高于对照组;与术前相比,术后90 d 2组患者的血清P1NP、β-CTX、OCN水平均降低,且试验组低于对照组(均P<0.05)。结论补肾壮骨方可有效缓解高龄股骨粗隆间骨折内固定术后患者的临床症状,改善髋关节功能,加速骨折恢复,且疗效确切。

Objective To observe the effect of Bushen Zhuanggu recipe on joint function and bone metabolism in elderly patients with intertrochanteric fracture after operation.Methods Sixty-six elderly patients with intertrochanteric fracture after internal fixation admitted to the department of orthopedics of Yuhang District Hospital of Traditional Chinese Medicine from January 2017 to June 2019 were selected and divided into control group and treatment group according to the random number table method,with 33 patients in each group.The control group was treated with Calcine D,while the experimental group was treated with Bushen Zhuanggu decoction,with one course of treatment for 3 weeks,and both groups were treated with 3 courses of treatment.The clinical efficacy of the two groups was compared 3 months after surgery,and the hip joint functional VAS and Harris scores were compared before and 30 days after surgery.Changes of serum P1NP,β-CTX and OCN before and 90 d after operation were compared.Results After 3 months of treatment,the effective rate of the control group was 66.67%,which was lower than that of the treatment group 84.85%.Compared with that before surgery,VAS score in both groups was decreased 7 days after surgery,and the control group was higher than the treatment group,while Harris score was increased,and the control group was lower than the treatment group.Compared with those before surgery,serum levels of P1NP,β-CTX and OCN in both groups 90 days after surgery,and the levels of P1NP,β-CTX and OCN in the control group were higher than those in the treatment group(all P<0.05).Conclusion Bushen Zhuanggu recipe can effectively relieve clinical symptoms,improve hip function and accelerate fracture recovery in elderly patients after internal fixation of intertrochanteric fracture.
