临床药师干预骨科大手术患者预防深静脉血栓的效果

Effect of clinical pharmacist intervention on prevention of deep venous thrombosis in patients undergoing major orthopedic surgery

目的观察临床药师干预骨科大手术患者预防深静脉血栓(DVT)的效果。方法回顾性分析2017年2月-2019年2月永新县人民医院内收治的接受骨科大手术患者200例的临床资料,根据术后是否接受临床药师干预分为观察组和对照组,各100例。对照组患者术后接受DVT常规性预防措施,无临床药师介入干预;观察组在对照组基础上排除大手术患者药物禁忌证后,实施全程临床药师干预。比较2组患者DVT风险评估情况、DVT发生率、预防措施实施情况。结果观察组的DVT风险安全率为97.00%,显著高于对照组的80.00%(χ^(2)=14.198,P=0.000);观察组DVT发生率为8.00%,显著低于对照组的21.00%(χ^(2)=6.816,P=0.009);观察组预防措施总实施率为100.00%,显著高于对照组的88.00%(χ^(2)=38.298,P=0.000)。结论临床药师干预对骨科大手术患者DVT的预防效果显著,能够有效降低DVT发生率,提高DVT评估情况,提高基础预防措施使用率,改善患者预后并提高其生活质量,值得临床推广应用。

Objective To observe the effect of clinical pharmacists on prevention of deep venous thrombosis(DVT in patients undergoing orthopedic surgery.Methods The clinical data of 200 patients with major orthopedic surgery in People’s Hospital of Yongxin County from February 2017 to February 2019 were retrospectively analyzed.They were divided into observation group and control group,100 cases in each group.The patients in the control group received routine preventive measures of DVT after operation without intervention of clinical pharmacists.The patients in the observation group were given the whole course of clinical pharmacists’intervention after excluding drug contraindications of major surgery patients on the basis of the control group.The risk assessment of DVT,the incidence of DVT and the implementation of preventive measures were compared between the two groups.Results The DVT risk safety rate of the observation group was 97.00%,which was significantly higher than 80.00%of the control group(χ^(2)=14.198,P=0.000);The incidence of DVT in the observation group was 8.00%,which was significantly lower than 21.00%in the control group(χ^(2)=6.816,P=0.009);The total implementation rate of preventive measures in the observation group was 100.00%,which was significantly higher than 88.00%in the control group(χ^(2)=38.298,P=0.000).Conclusion Clinical pharmacist intervention has a significant preventive effect on DVT in patients with major orthopedic surgery,which can effectively reduce the incidence of DVT,improve the evaluation of DVT,improve the utilization rate of basic preventive measures,improve the prognosis of patients and improve their quality of life,which is worthy of clinical application.
