吸入糖皮质激素联合多索茶碱治疗老年哮喘的效果及对外周血单个核细胞中肿瘤坏死因子-α诱导蛋白8样因子-2组织因子的影响

目的探讨吸入糖皮质激素(ICS)联合多索茶碱治疗老年哮喘的效果及对外周血单个核细胞(PBMCs)肿瘤坏死因子-α诱导蛋白8样因子-2(TIPE2)、组织因子(TF)的影响。方法选取2017年2月至2018年12月老年哮喘患者141例分为观察组68例和对照组73例,对照组给予ICS联合长效2受体激动剂治疗,观察组给予ICS联合多索茶碱治疗,观察2组治疗前后肺功能、PMBCs中TIPE2、TF表达等。结果治疗后第1秒用力呼气容积(FEV1)、用力肺活量(FVC)和最大呼气流速峰值(PEF),观察组分别为(2.2±0.7)L、(3.7±0.8)L和(3.2±0.8)L/s,高于对照组(P<0.05);治疗后哮喘测试控制(ACT)评分,观察组为(27±6)分,高于对照组(22±7)分(P<0.05);治疗后PBMCs中TIPE2相对表达量,观察组为(0.66±0.21),高于对照组(0.53±0.17)(P<0.05),而TF相对表达量为(0.22±0.09),低于对照组(0.30±0.10)(P<0.05)。结论ICS联合多索茶碱治疗老年哮喘有较好的效果,改善患者肺功能,升高PBMCs中TIPE2表达,降低TF表达。

Objective To investigate the effect of inhaled corticosteroids(ICS)combined with doxofylline in the treatment of senile asthma and the effect of tumor necrosis factor inducible protein 8-like factor-2(TIPE2)and tissue factor(TF)in peripheral blood mononuclear cells(PBMCs).Methods One hundred and forty-one elderly asthmatics patients from February 2017 to December 2018 were divided into observation group(n=68)and control group(n=73);according to the treatment plan,the control group was treated with ICS combined with long-actingβ2 receptor agonist,the observation group was treated with ICS combined with theophylline,and the lung function,the expression of TIPE2 and TF in PMBCs before and after treatment and so on were observed.Results The forced expiratory volume at the first second(FEV1),forced vital capacity(FVC)and maximum expiratory velocity(PEF)in the observation group after treatment were(2.2±0.7)L,(3.7±0.8)L and(3.2±0.8)L/s,significantly higher than those in the control group(P<0.05);The asthma test control(ACT)score of the observation group was(27±6),outstripping considerably that of the control group(P<0.05);The relative expression of TIPE2 in PBMCs of the observation group was(0.66±0.21),significantly higher than that of the control group(0.53±0.17)(P<0.05),while that of TF was(0.22±0.09),significantly lower than that of the control group(0.30±0.10)(P<0.05).Conclusion ICS combined with doxofylline can effectively cure asthma in the elderly group,can improve the lung function of the patients,enhance the expression of TIPE2 and impede the expression of TF in PBMCs.
