摘要
目的:探讨血CRP、UA、α1-MG和尿α1-MG监测老年原发性高血压患者早期肾损伤的价值。方法:选择2019年1月~10月于某院就诊的老年原发性高血压患者120例为研究对象,根据其内生肌酐清除率(Ccr)分为肾病高血压组(Ccr<80ml/min,63例)和单纯高血压组(Ccr≥80ml/min,57例),同时选择60例参加体检的健康老人为对照组,观察比较3组血尿素氮(BUN)、肌酐(Cr)、C-反应蛋白(CRP)、尿酸(UA)、α1-微球蛋白(α1-MG)和尿α1-MG。结果:肾病高血压组血BUN、Cr、CRP、UA、α1-MG和尿α1-MG均高于其它两组,差异具有统计学意义(P<0.05);单纯高血压组CRP、UA、α1-MG和尿α1-MG显著高于对照组,差异具有统计学意义(P<0.05)。结论:血CRP、UA、α1-MG和尿α1-MG在监测老年原发性高血压患者早期肾损伤方面具有临床价值。
Objective:To investigate the value of serum CRP,UA,α1-MG and urineα1-MG in monitoring early renal injury in elderly patients with essential hypertension.Methods:120 elderly patients with essential hypertension in a hospital from January to October 2019 were selected as the research objects.According to their endogenous creatinine clearance rate(CCR),they were divided into nephrotic hypertension group(Ccr<80ml/min,63 cases)and simple hypertension group(Ccr≥80ml/min,57 cases).At the same time,60 healthy elderly people who participated in physical examination were selected as the control group.Blood urea nitrogen(BUN),creatinine(Cr),C-reactive protein(CRP),uric acid(UA),α1-microglobulin(α1-MG)and urinaryα1-MG were observed and compared among the three groups.Results:The BUN,Cr,CRP,UA,α1-MG in blood and urineα1-MG were significantly higher in nephrotic hypertension group than those in other two groups,and the difference was statistically significant(P<0.05).CRP,UA,α1-MG andα1-MG in urine were significantly higher in simple hypertension group than those in control group,and the difference was statistically significant(P<0.05).Conclusion:Serum CRP,UA,α1-MG and urineα1-MG have clinical value in monitoring early renal injury in elderly patients with essential hypertension.
