中药右归饮加减联合糖皮质激素对系统性红斑狼疮肾功能、中医症状总积分的影响

Effect of TCM Modified Youguiyin(右归饮)Combined with Glucocorticoid on Renal Function and Total TCM Symptoms Scores in Patients with SLE

目的研究分析中药右归饮加减联合糖皮质激素对系统性红斑狼疮(systemic lupus erythematosus,SLE)患者肾功能、中医症状总积分的影响。方法选取2014年1月—2018年6月医院中西医结合科127例患者作为研究对象,按照随机数字法对患者进行分组,其中采用中药右归饮加减联合糖皮质激素治疗的67例患者作为观察组,采用标准激素治疗的60例患者作为对照组。比较两组患者治疗前以及治疗后2周、4周的形体评分,记录并比较两组患者治疗前以及治疗后4周的血液免疫学、肾功能以及中医症状总积分。结果治疗2周后以及治疗4周后两组形体评分明显高于治疗前(P<0.05);治疗4周后观察组形体评分明显高于对照组(P<0.05)。治疗后两组CRP明显低于治疗前,C3、C4以及CH50明显高于治疗前(P<0.05);治疗后观察组CRP明显低于对照组,C3、C4以及CH50明显高于对照组(P<0.05)。治疗4周后两组Scr、BUN以及24 h尿蛋白明显低于治疗前(P<0.05);治疗4周后观察组Scr、BUN以及24 h尿蛋白明显低于对照组(P<0.05)。治疗4周后两组皮肤斑疹、眩晕耳鸣、腰膝酸软以及肢体疼痛明显低于治疗前,治疗4周后观察组皮肤斑疹、眩晕耳鸣、腰膝酸软以及肢体疼痛明显低于对照组(P<0.05)。结论中药右归饮加减联合糖皮质激素治疗能够改善SLE患者肾功能以及中医症状总积分,提高治疗效果。

Objective To analyze the effect of TCM Modified Youguiyin(右归饮)combined with glucocorticoid on renal function and total TCM symptoms score in patients with systemic lupus erythematosus(SLE).Methods From January 2014 to June 2018,127 patients from the Department of Integrated Traditional Chinese and Western Medicine of our hospital were selected as the research subjects.The patients were grouped according to the random number method.Among them,67 patients treated with Youguiyin and glucocorticoids were used as the observation group.Sixty patients treated with standard hormones were used as the control group.The body scores of the two groups were compared before treatment and 2 and 4 weeks after treatment,and the total scores of hemoimmunology,renal function and TCM symptoms were recorded and compared before and after treatment.Results After 2 and 4 weeks after treatment,the body shape score of the two groups was significantly higher than that before treatment(P<0.05).After 4 weeks of treatment,the body shape score of the observation group was significantly higher than that of the control group(P<0.05).After treatment,CRP in the two groups was significantly lower than that before treatment,and C3,C4 and CH50 were significantly higher than those before treatment(P<0.05).After treatment,CRP in the observation group was significantly lower than that in the control group,while C3,C4 and CH50 were significantly higher than those in the control group(P<0.05).After 4 weeks of treatment,Scr,BUN and urinary protein in the two groups were significantly lower than those before treatment(P<0.05).After 4 weeks of treatment,Scr,BUN and urinary protein in the observation group were significantly lower than those in the control group(P<0.05).After 4 weeks of treatment,skin macular rash,vertigo tinnitus,lumbar and knee tenderness and limb pain in the two groups were significantly lower than those before treatment.After 4 weeks of treatment,skin macular rash,vertigo tinnitus,lumbar and knee tenderness and limb pain in the observation group were significantly lower than those of the control group(P<0.05).Conclusions TCM Modified Youguiyin combined with glucocorticoid can improve the renal function and the total TCM symptoms scores in TLE patients and improve the therapeutic effect.
