会阴侧切对初产妇性功能及生活质量的影响

Effects of Lateral Episiotomy on Sexual Function and Quality of Life in Primiparas

目的研究会阴侧切对初产妇产后性功能恢复以及生活质量的影响。方法选取2018年6月至2019年10月在本院分娩的初产妇200名,采用随机数表法分为观察组和对照组,各有100名产妇。其中观察组产妇实施会阴侧切顺产分娩;对照组产妇采取自然顺产。观察两组产后的性功能(FSFI)、生活质量评分(GQOL-74)以及并发症情况。结果观察组术后2个月、4个月、6个月的性功能情况、GQOL-74评分较对照组较差(P<0.05);观察组产后产妇的盆底账期脱垂、压力性尿失禁情况和对照组并无差别(P>0.05);观察组产妇的性功能障碍发生率和会阴水肿率高于对照组(P<0.05)。结论会阴侧切分娩对产妇的盆底脏器脱垂、压力性尿失禁影响不大,但会增加产妇产后水肿发生率,对产妇的性功能和性生活有严重的影响,临床分娩因引导产妇自然生产,避免会阴侧切降低产妇的生活质量。

Objective To study the effects of lateral episiotomy on postpartum recovery of sexual function and quality of life in primiparas.Methods During the period from June 2018 to October 2019,200 primiparas who were delivered in the hospital were enrolled.They were divided into observation group and control group by random number table method,100 cases in each group.The observation group underwent lateral episiotomy,while control group underwent natural delivery.The scores of Female Sexual Function Index(FSFI)and Generic Quality of Life Inventory-74(GQOL-74),and complications in both groups were observed.Results At 2 months,4 months and 6 months after surgery,sexual function status and GQOL-74 scores in observation group were worse than those in control group(P<0.05).There were no significant differences in postpartum pelvic floor prolapse and stress urinary incontinence between the two groups(P>0.05).The incidence rates of sexual dysfunction and perineal edema in observation group were higher than those in control group(P<0.05).Conclusion The effects of lateral episiotomy are fewer on pelvic floor prolapse and stress urinary incontinence.However,it will increase the incidence of postpartum edema,which is of serious effects on maternal sexual function and sexual life.In clinical delivery,natural delivery should be applied to avoid decreased quality of life induced by lateral episiotomy.
