中医辨治高原睡眠障碍

Syndrome Differentiation and Treatment of Sleep Disorder at High Altitude by TCM

缺氧、干燥、高寒是高原睡眠障碍重要病因,清气不足、气虚血瘀是其重要病机,辨治首先从虚实入手,以虚证为主者补气扶阳,可用朱砂安神丸、天王补心丹、归脾丸调补心脉气血,配伍百合、莲子、柏子仁养心安神、补养心阴;清气不足者用补中益气汤益气补中;阴血亏虚者用生地黄、熟地黄和百合地黄汤。实证为主者清热化痰活血,属痰热内扰者,治以黄连温胆汤;属寒凝血瘀者,治以血府逐瘀汤;兼见气虚或寒凝证候,给予补气、温阳药物。

Hypoxia,dryness and high cold are the important causes of sleep disorder at high altitude.Insufficient lucid qi,qi deficiency and blood stasis are the important pathogenesis.The first point of syndrome differentiation and treatment is deficiency and excess.For those with deficiency syndrome,Cinnabar Spirit-Calming Pills,Celestial Emperor Heart-Supplementing Elixir,and Spleen-Restoring Pills can be used to regulate and nourish qi and blood of heart vessel;Baihe(Bulbus Lilii),Lianzi(Semen Nelumbinis) and Baiziren(Semen Platycladi) can be combined to nourish the heart and calm spirit,and nourish heart yin.For insufficient lucid qi,Center-Supplementing and Qi-Boosting Decoction is used to boost qi and supplement middle energizer.For deficient yin blood,Shengdihuang(Radix Rehmanniae),Shudihuang(Radix Rehmanniae Praeparata) and Lily Bulb and Rehmannia Decoction can be used.For excess syndrome,the treatment should focus on clearing heat,resolving phlegm and promoting blood circulation.For phlegm-heat disturbing inside the body,Coptis Gallbladder-Warming Decoction can be used.For syndrome of congealing cold with blood stasis,Blood Mansion Stasis-Expelling Decoction can be used.For those combined with qi deficiency or congealing cold,the medicinals for nourishing qi and warming yang can be used.
