茵栀黄口服液辅助肝门空肠吻合术治疗新生儿胆道闭锁的临床效果

Clinical Effects of Yinzhihuang Oral Liquid Combined with Kasai Portoenterostomy in the Treatment of Neonatal Biliary Atresia

目的:探讨茵栀黄口服液辅助肝门空肠吻合术治疗新生儿胆道闭锁的临床效果。方法:选择2015年3月至2018年1月我院收治并确诊的胆道闭锁新生儿120例。按随机字母表法将新生儿分为观察组和对照组,每组60例。对照组采用肝门空肠吻合术治疗,观察组采用茵栀黄口服液联合肝门空肠吻合术治疗,比较两组的疗效、血生化指标[总胆红素(TBIL)、天冬氨酸氨基转移酶(AST)和丙氨酸氨基转移酶(ALT)]、肝纤维化指标[层粘连蛋白(LN)、透明质酸(HA)、Ⅲ型前胶原(PC-Ⅲ)和Ⅳ型胶原(Ⅳ-C)]、细胞因子[白细胞介素-18(IL-18)、肿瘤坏死因子-α(TNF-α)和转化生长因子-β(TGF-β)]和不良反应。结果:治疗1年后,观察组有效率90.00%,高于对照组的75.00%(P<0.05)。治疗1年后,观察组的TBIL、AST、ALT、LN、HA、PC-Ⅲ、Ⅳ-C、IL-18、TNF-α和TGF-β水平均低于对照组(P<0.05)。两组的不良反应发生率比较差异无统计学意义(P>0.05)。结论:与肝门空肠吻合术单独治疗相比,茵栀黄口服液辅助肝门空肠吻合术治疗新生儿胆道闭锁的疗效更好。

Objective:To probe into the clinical effects of Yinzhihuang oral liquid combined with Kasai portoenterostomy in the treatment of neonatal biliary atresia.Methods:Totally 120 neonates with biliary atresia admitted to our hospital from Mar.2015 to Jan.2018 were selected to be divided into the observation group and the control group via the random number table,with 60 cases in each group.The control group was given Kasai portoenterostomy,while the observation group received Yinzhihuang oral liquid combined with Kasai portoenterostomy.Efficacy,blood biochemical indicators such as total bilirubin(TBIL),aspartic transaminase(AST)and alanine aminotransferase(ALT),liver fibrosis indicators such as laminin(LN),hyaluronic acid(HA),procollagenⅢ(PC-Ⅲ)and typeⅣcollagen(Ⅳ-C),cytokines such as interleukin(IL)-18,tumour necrosis factor(TNF)-αand transforming growth factor(TGF)-βand adverse drug reactions of two groups were compared.Results:After one year of treatment,the effective rate of the observation group was 90.00%,higher than 75.00%of the control group(P<0.05).After one year of treatment,the levels of TBIL,AST,ALT,LN,HA,PC-Ⅲ,Ⅳ-C,IL-18,TNF-αand TGF-βin the observation group were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse drug reactions between two groups(P>0.05).Conclusion:Compared with Kasai portoenterostomy alone,the efficacy of Yinzhihuang oral liquid combined with Kasai portoenterostomy in the treatment of neonatal biliary atresia is more remarkable.
