应用VFM技术评价AMI患者PCI术前术后左室涡流变化特点

Evaluation of left ventricular eddy current in patients with AMI before and after PCI by VFM

目的:使用VFM技术分析AMI患者PCI术前术后左室涡流改变。方法:从我院选取2019年8月至2020年8月期间接受经皮冠状动脉介入术(PCI)的急性心肌梗死(AMI)患者32例为实验的观察组,另选取同期健康体检者32例作为对照组,分析两组对象超声心动图检查结果,并使用血流向量成像技术(VFM)技术分析急性心肌梗死患者PCI术前术后左室涡流改变。结果:观察组PCI术前LVESD、LVEDD、LVESV、LVEDV均高于对照组(P<0.05),LVEF低于对照组(P<0.05);术后检查各指标与术前(P<0.05),与对照组(P>0.05);术前纵径大于对照组(P<0.05);涡流强度、最大向量速度、流量等低于对照组(P<0.05),术后4周纵径、涡流强度、最大向量速度、流量等与对照组(P>0.05),术后4周纵径低于术前(P<0.05),涡流强度、最大向量速度、流量等高于术前(P<0.05)。结论:健康体检者体内左心室收缩期涡流具有一定规律,急性心肌梗死患者体内左心室收缩期涡流规律紊乱,经皮冠状动脉介入术可有效改善此类患者体内左心室收缩期涡流情况。

Objective:To analyze the changes of left ventricular vortex before and after PCI in AMI patients using VFM technology.Methods:From our hospital,32 patients with acute myocardial infarction(AMI)who underwent percutaneous coronary intervention(PCI)between August 2019 and August 2020 were selected as the experimental observation group,and 32 healthy patients were selected during the same period.As a control group,the results of echocardiography of the two groups were analyzed,and the changes of left ventricular vortex flow before and after PCI in patients with acute myocardial infarction were analyzed using vector blood flow imaging(VFM)technology.Results:The LVESD,LVEDD,LVESV,and LVEDV of the observation group before PCI were higher than those of the control group(P<0.05),and LVEF was lower than that of the control group(P<0.05);each index of postoperative examination was compared with that of preoperative(P<0.05).Compared with the control group(P>0.05);the longitudinal diameter before operation is larger than the control group(P<0.05);the vortex intensity,maximum vector velocity,flow,etc.are lower than the control group(P<0.05),the longitudinal diameter and vortex intensity are 4 weeks after operation,Maximum vector velocity,flow,etc.compared with the control group(P>0.05),4 weeks after operation,the longitudinal diameter was lower than before operation(P<0.05),vortex intensity,maximum vector velocity,flow,etc.were higher than before operation(P<0.05).Conclusion:The left ventricular systolic vortex in healthy subjects has a certain regularity,and the left ventricular systolic vortex is disordered in patients with acute myocardial infarction.Percutaneous coronary intervention can effectively improve the left ventricular systolic vortex in such patients.
