摘要
目的运用实时荧光定量PCR(qRT-PCR)法检测慢性乙型肝炎(CHB)患者循环血清中miRNA-122水平的相对表达量,分析其在CHB患者循环血清中的表达水平及与HBV DNA载量和CHB患者病情程度的相关性。方法选取2019年3月至2020年3月于我院就诊的100例CHB患者为研究对象,以25例乙肝病毒携带者、30例HBV相关性肝硬化代偿期患者、20例健康体检者为对照组研究对象,所有研究对象均采用qRT-PCR法检测miRNA-122表达水平的相对表达量。结果CHB组患者循环血清中miRNA-122水平的相对表达量较健康体检组过表达,差异有统计学意义(P<0.01);乙肝病毒携带者组血清miRNA-122水平的相对表达量较乙肝肝硬化组比较差异无统计学意义(P>0.05);不同病情程度的CHB患者随病情加重,其miRNA-122水平相对表达量逐渐高表达,较乙肝病毒携带者组比较差异有统计学意义(F=249.25,P<0.01);乙肝携带者组、CHB组及乙肝肝硬化组患者血清中miRNA-122表达水平与HBV DNA载量间均呈正相关系(r=0.323,P=0.031;r=0.484,P=0.000,r=0.179,P=0.033);CHB患者血清中miRNA-122水平的相对表达量较HBsAg、HBeAg水平呈正相关系(r=0.335,P=0.000;r=0.314,P=0.000);CHB患者血清中miRNA-122水平的相对表达量与HBsAb、HBeAb、HBcAb水平比较无相关性(r=0.123,P=0.141;r=0.167,P=0.450;r=0.019,P=0.821);CHB患者血清中miRNA-122的表达水平与TBil、ALT水平呈正相关系(r=0.903,P=0.000;r=0.799,P=0.000)。结论CHB患者血清miRNA-122与HBV DNA载量、TBil、ALT水平关系密切,有助于CHB病情程度判定及抗HBV治疗终点时机选择与病情复发评估的指标之一。
Objective Quantitative real-time PCR(qRT-PCR)was used to detect the relative expression levels of miRNA-122 in circulating serum for chronic hepatitis B(CHB)patients to analyze their correlation with HBV DNA load and CHB disease severity.Methods A total of 100 CHB patients admitted to the Affiliated Hospital of North Sichuan Medical College from March 2019 to March 2020 were selected as the study subjects.25 hepatitis B virus carriers,30 HBV-related cirrhosis patients,and 20 healthy physical examination subjects were selected as the control group.qRT-PCR was used to detect the relative expression level of miRNA-122 in all the study subjects.Results The relative expression level of miRNA-122 in circulating serum of patients in chronic hepatitis B group was higher than that in healthy group,and the difference was statistically significant(P<0.01);The relative expression level of miRNA-122 in serum of hepatitis B virus carrier group was not statistically significant compared with that of hepatitis B cirrhosis group(P>0.05).With the aggravation of chronic hepatitis B patients with different severity of disease,the relative expression level of miRNA-122 gradually increased,and the difference was statistically significant compared with the hepatitis B virus carrier group(F=249.25,P<0.01);The expression level of miRNA-122 in serum of CHB patients was positively correlated with HBV DNA load in hepatitis B carrier group,chronic hepatitis B group and cirrhosis group(r=0.323,P=0.031;r=0.484,P=0.000,r=0.179,P=0.033).The relative expression level of miRNA-122 in serum of patients with chronic hepatitis B was positively correlated with HBsAg and HBeAg levels(r=0.335,P=0.000;r=0.314,P=0.000);The relative expression level of miRNA-122 in serum of CHB patients was not correlated with HBsAb,HBeAb and HBcAb levels(r=0.123,P=0.141;r=0.167,P=0.450;r=0.019,P=0.821);Serum levels of miRNA-122 and TBil,ALT in patients with chronic hepatitis B were positively correlated(r=0.903,P=0.000;r=0.799,P=0.000).Conclusion Serum miRNA-122 in CHB patients is closely related to HBV DNA load and TBil,ALT level,which is helpful for the determination of the severity of chronic hepatitis B,the timing of the end point of anti-HBV treatment and the evaluation of disease recurrence.
