低分子肝素钙治疗肺心病并呼吸衰竭的临床疗效研究

Study on the Clinical Efficacy of Low Molecular Weight Heparin Calcium in the Treatment of Pulmonary Heart Disease with Respiratory Failure

目的研究肺心病并法呼吸衰竭采用低分子肝素钙治疗的临床疗效。方法选该院2019年4月—2020年4月期间接收的84例肺心病合并呼吸衰竭患者为研究对象,依照随机数表均分成参考组和研究组,每组42例患者。其中,参考组采取常规治疗,研究组采取低分子肝素钙治疗,观察两组患者动脉血气指标、血液流变学指标的变化以及临床治疗疗效。结果通过治疗后分析,研究组患者PaO2(61.60±1.88)mmHg、PaCO2(78.20±14.59)mmHg等动脉血气指标对比参考组,差异有统计学意义(P<0.05),pH值对比,差异无统计学意义(P>0.05);研究组患者纤维蛋白原(3.22±1.12)g/L、血细胞容比(0.38±0.03)%、全血高切黏度(5.45±0.07)mPa·s等指标对比参考组,差异有统计学意义(P<0.05);研究组治疗有效率(90.48%)明显优于参考组(71.43%),差异有统计学意义(χ2=4.941,P<0.05)。结论在肺心病合并呼吸衰竭患者中采用低分子肝素钙治疗临床疗效突出,能够改善患者心肺功能、通气状况、血液性状,提高患者生存期,具有显著的临床推广应用价值。

Objective To study the clinical efficacy of low-molecular-weight heparin calcium treatment for pulmonary heart disease complicated with respiratory failure.Methods The 84 patients with pulmonary heart disease and respiratory failure received in the hospital from April 2019 to April 2020 were selected as the research objects,and were divided into the reference group and the study group according to the random number table,with 42 patients in each group.Among them,the reference group was treated with conventional treatment,and the study group was treated with low-molecular-weight heparin calcium,and the changes in arterial blood gas indexes,hemorheology indexes and clinical treatment effects of the two groups were observed.Results Through post-treatment analysis,there were statistically significant differences in arterial blood gas indexes such as PaO2(61.60±1.88)mmHg and PaCO2(78.20±14.59)mmHg compared to the reference group(P<0.05),and the pH value was compared and no statistically significant differences were found(P>0.05);there were statistically significant differences in fibrinogen(3.22±1.12)g/L,hematocrit ratio(0.38±0.03)%,whole blood high shear viscosity(5.45±0.07)mPa·s and other indicators compared to the reference group,the difference was statistically significant(P<0.05);the treatment effective rate of the study group(90.48%)was significantly better than that of the reference group(71.43%),and the difference was statistically significant(χ2=4.941,P<0.05).Conclusion Low-molecular-weight heparin calcium treatment in patients with cor pulmonale and respiratory failure has a prominent clinical effect,which can improve the patient's cardiopulmonary function,ventilation status,blood characteristics,and improve the survival of patients.It has significant clinical application value.
