膝八针配合独活寄生汤加味治疗原发性膝关节骨性关节炎的效果

Effect of Xiba Acupuncture combined with modified Duhuo Jisheng Decoction on primary knee osteoarthritis

目的探讨膝八针配合独活寄生汤加味治疗原发性膝关节骨性关节炎的效果。方法选取2019年9月~2020年9月信阳市中心医院收治的114例原发性膝关节骨性关节炎患者,按照随机数字表法分为对照组(57例)与研究组(57例)。对照组采用常规西药治疗,研究组采用膝八针配合独活寄生汤加味治疗。比较两组的临床疗效、炎症因子、关节滑液相关细胞因子水平、膝关节功能及不良反应发生情况。结果研究组的治疗总有效率为91.22%,高于对照组的77.19%,差异有统计学意义(P<0.05)。治疗后,两组的白细胞介素-6(IL-6)、肿瘤坏死因子(TNF-α)、超敏C-反应蛋白(hs-CRP)水平低于治疗前,且研究组的各项炎症因子水平低于对照组,差异有统计学意义(P<0.05)。治疗后,两组的骨桥蛋白(OPN)、血管内皮生长因子(VEGF)水平低于治疗前,骨保护素(OPG)水平高于治疗前,且研究组的OPN、VEGF水平低于对照组,OPG水平高于对照组,差异有统计学意义(P<0.05)。研究组的膝关节功能优良率为85.96%,高于对照组的70.18%,差异有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论应用膝八针配合独活寄生汤加味治疗原发性膝骨性关节炎的效果显著,可改善关节滑液相关细胞因子水平,降低炎症因子水平,有利于膝关节功能的恢复,值得推广。

Objective To investigate the effect of Xiba Acupuncture combined with modified Duhuo Jisheng Decoction on primary knee osteoarthritis.Methods A total of 114 patients with primary knee osteoarthritis admitted to Xinyang Central Hospital from September 2019 to September 2020 were selected and divided into control group(57 cases)and study group(57 cases)according to random number table method.The control group was treated with conventional western medicine,and the study group was treated with Xiba Acupuncture combined with modified Duhuo Jisheng Decoction.Clinical efficacy,inflammatory cytokines,levels of synovial fluid related cytokines,knee function and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the study group was 91.22%,higher than that of the control group(77.19%),and the difference was statistically significant(P<0.05).After treatment,the levels of interleukin-6(IL-6),tumor necrosis factor(TNF-α)and hypersensitive C-reactive protein(hs-CRP)in the two groups were lower than before treatment,and the levels of various inflammatory factors in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of osteopontin(OPN)and vascular endothelial growth factor(VEGF)in the two groups were lower than before treatment,and the level of osteoprotective hormone(OPG)were higher than before treatment,and the levels of OPN and VEGF in the study group were lower than those of the control group,and OPG was higher than that of the control group,the differences were statistically significant(P<0.05).The excellent and good rate of knee function in the study group was 85.96%,which was higher than that in the control group(70.18%),and the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The application of Xiba Acupuncture combined with modified Duhuo Jisheng Decoction in the treatment of primary knee osteoarthritis has significant effects,which can improve the level of cytokines related to synovial fluid,reduce the level of inflammatory factors,and is conducive to the recovery of knee function.It is worth popularizing.
