医院-社区一体化服务模式在日间手术患者康复路径中的构建与现状分析

The Construction and Status Analysis of the Integrated Hospital-community Service Model in the Rehabilitation Path of Day Surgery Patients

目的研究日间手术患者康复路径中医院-社区一体化服务模式的构建与现状分析。方法成都市温江区中医医院于2019年6月开始应用医院-社区一体化服务模式,采用文献调查法+横断面调查+德尔菲专家咨询法构建医联体日间手术康复路径,分析医联体单位工作人员的培训情况与一体化服务模式的应用情况。结果2019年6月一2020年9月共举办了5次医联体单位工作人员培训,共计培训43人次。参加培训的医联体单位工作人员中97.67%认为该次培训对转诊方式优化有帮助,86.05%认为自己了解医院-社区一体化服务模式工作流程,97.67%认为该次培训对进行随访非常有帮助,100.00%认为该次培训对伤口护理非常有帮助,且95.35%的医联体单位工作人员表示满意。日间手术病房下转社区随访提供康复指导的患者共762例,社区医院上转患者共103例,保证了患者出院后的服务质量。结论医院-社区一体化服务模式对于优化日间手术的预约流程,调动国家分级诊疗制度的落实等有十分重要的意义。

Objective To study the construction and current analysis of the integrated service model of Chinese hospital-community in the rehabilitation path of day surgery patients.Methods The Wenjiang District Traditional Chinese Medicine Hospital of Chengdu began to apply the hospital-community integrated service model in June 2019,using the literature survey method+cross-sectional survey+Delphi expert consultation method to construct the medical consortium day surgery rehabilitation path,and analyze the medical consortium training situation of the unit staff and the application situation of the integrated service model.Results From June 2019 to September 2020,a total of 5 medical consortia unit staff trainings were held,and a total of 43 people were trained.97.67%of the staff of the medical consortium who participated in the training believed that the training was helpful to the optimization of referral methods,86.05%believed that they understood the workflow of the hospital-community integrated service model,and 97.67%thought the training was very useful for follow-up,100.00%thought this training was very helpful for wound care,and 95.35%of the staff of the medical consortium were satisfied.A total of 762 patients were transferred from the day surgery ward to community follow-up to provide rehabili tation guidance,and a total of 103 patients were transferred from community hospitals to ensure the quality of service after the patients were discharged from the hospital.Conclusion The hospital-community integrated service model is of great significance for optimizing the appointment process of day surgery and mobilizing the implementation of the national hierarchical diagnosis and treatment system.
