目的探讨网络模式在社区老年慢性病健康管理中的应用效果。方法结合社区老年慢性病健康管理的现状进行分析,探究网络模式在健康管理中的应用价值,观察网络模式下健康教育、病情监测情况,评价“互联网+医养结合”的实施效果,针对慢性病健康管理网络模式所面临的困境,寻找有效的解决办法。结果网络模式在社区老年慢性病健康管理中的应用,有利于健康教育的顺利开展,增加社区老年慢性病患者对于疾病的认知,提高其自我护理能力。网络模式下的病情监测,能够实时掌握患者的病情变化,并结合患者的实际情况,制定个体化的护理方案。“互联网+医养结合”的有效开展,则增进了医院、社区、家庭之间的联系,实现院内治疗和家庭护理的良好衔接,为老年患者提供居家护理服务和社会支持。结论在社区老年慢性病健康管理中,应该重点加强对慢性病的防治。网络模式在慢性病健康管理中的应用,可以更加长效、稳定地控制血糖、血压水平,减少病情复发频次,提高患者的生活质量。

Objective To explore the application effect of the network model in the community health management of elderly chronic diseases.Methods Analyze the status quo of health management for elderly chronic diseases in the community,explore the application value of the network model in health management,observe the situation of health education and disease monitoring under the network model,evaluate the effect of the implementation of"Internet+medical care",and target chronic disease health management,to find effective solutions to the difficulties faced by the network model.Results The application of the network model in the health management of elderly chronic diseases in the community is conducive to the smooth development of health education,increasing the awareness of the elderly patients with chronic diseases in the community on the disease and improving their self-care ability.Condition monitoring under the network mode can grasp the patient's condition changes in real time,and combine the actual situation of the patient to formulate an individualized nursing plan.The effective development of"Internet+medical and nursing care"has enhanced the connection between hospitals,communities,and families,realized a good connection between in-hospital treatment and home care,and provided home care services and social support for elderly patients.Conclusion In the community health management of chronic diseases for the elderly,we should focus on strengthening the prevention and treatment of chronic diseases.The application of the network model in the health management of chronic diseases can control blood glucose and blood pressure levels more long-term and stably,reduce the frequency of relapses,and improve the quality of life of patients.
