突发性耳聋患者高刺激率听性脑干反应检测对预后的预测作用

Predictive effect of detection of auditory brainstem response with high stimulation rate on prognosis in patients with sudden deafness

摘要 目的针对突发性耳聋患者,采用高刺激率听性脑干反应检测方法,探讨其预测价值。方法本次研究对象所选取的研究对象,共有63例,其中40例是我收治的突发性耳聋患者为病例组,另外23例是同期筛选出来的听力正常群体为参照组,选取时间段为2019年2月至2020年9月。两组分别进行分别进行高、低刺激率ABR检测,比较波Ⅰ、Ⅲ、Ⅴ的潜伏期(peak latency,PL)和Ⅰ~Ⅲ、Ⅲ~Ⅴ和Ⅰ~Ⅴ峰间潜伏期(interpeak latency,IPL)及两种刺激速率PL差值(ΔPL)、IPL差值(ΔIPL)。听性脑干反应检测,注意观察检测结果。根据病例组的听性脑干反应主波引起率结果,分为甲、乙两组,分别是脑干反应结果引出者和未引出者,注意观察两组的治疗效果。结果低刺激频率下,健康组与病例组ABR波I、III、V的潜伏期相比无差异(P>0.05),高刺激频率下,I波潜伏期同参照组相比,病例组有明显延长(P<0.05),其余无异;低刺激频率下,健康组与病例组I~III波、III~IV波、I~V波的波间期无差异(P>0.05);高刺激频率下,同健康组相比,病例组I~V波的波间期有明显延长(P<0.05)其余无异;病例组PL值同健康组相比,病例组同健康组相比,在I波上ΔPL值相对较高(P<0.05),其余无异;病例组同健康组相比,在I~III波、III~IV波、I~V波上ΔPL值相对较高(P<0.05);病例组中甲组治疗有效率90.63%显著高于乙组37.50%,数据比较差异较大(χ^(2)=61.280,P<0.05)。结论运用听性脑干反应检测突发性耳聋,特别是高刺激率检测,可预测患者预后治疗效果,值得推广。 Objective To investigate the predictive value of auditory brainstem response detection with high stimulation rate in patients with sudden deafness.Methods A total of 63 subjects were selected for this study,among which 40 were patients with sudden deafness admitted to our hospital as the case group,and the other 23 were normal hearing groups screened out in the same period as the reference group,from February 2019 to September 2020.Respectively in the two groups of high and low stimulation rate ABR testsrespectively,comparing waveⅠ,Ⅲ,Ⅴthe incubation period(the peak latency,PL)andⅠ~Ⅲ,Ⅲ~ⅤandⅠ~Ⅴpeak to peak latency period(interpeak latency,IPL)and two kinds of stimulation rate PL difference(ΔPL),IPL difference(ΔIPL).Detection of auditory brainstem reaction,pay attention to the detection results.According to the results of the main wave induced rate of auditory brainstem response in the case group,the patients were divided into group A and group B,which were the subjects who had elicited the results of brainstem response and those who had not elicited the results.The therapeutic effect of the two groups was observed.Results Under low stimulation frequency,the incubation period of ABR wave I,III and V in the healthy group was not different from that in the case group(P>0.05).Under high stimulation frequency,the incubation period of I wave was significantly longer in the case group than in the control group(P<0.05),and the rest showed no difference.At low stimulation frequency,there was no difference between the healthy group and the case group in the wave interval of I~III,III~IV and I~V waves(P>0.05).Compared with the healthy group,the interwave period of i-V wave in the case group was significantly prolonged(P<0.05)under high stimulation frequency,and there was no difference.CPT PL value compared with healthy group,case group compared with healthy group,on the I waveΔPL value is relatively high(P<0.05),the same the rest;Case group compared with healthy group,in the wave,I~III III~IV,I~V wave onΔPL value is relatively high(P<0.05);In the case group,the effective rate of treatment in group A was 90.63%significantly higher than that in group B(37.50%),with a significant difference in data(χ^(2)=61.280,P<0.05).Conclusion The use of auditory brainstem response to detect sudden deafness,especially the detection of high stimulation rate,can predict the prognosis of patients with treatment,and is worthy of promotion.
