冠向复位瓣治疗MillerⅠ类单牙牙龈退缩的临床研究

Treatment of MillerⅠgingival recession of single tooth with coronally advanced flap procedure

目的:观察冠向复位瓣手术用于治疗单牙牙龈退缩的临床效果。方法:纳入12例单牙牙龈退缩患者,均采用冠向复位瓣手术,术前、术后3个月、术后6个月分别记录患牙探诊深度(pocket depth,PD)、牙龈退缩高度(gingival recession height,RGH)和临床附着丧失(clinical attachment level,CAL);计算术后3个月、6个月的根面覆盖率(root coverage rate,RC);采用根面覆盖美学评分系统(root coverage esthetics,RES)进行效果评价。结果:术前、术后PD值无明显变化;RGH术后3个月、6个月分别为(0.63±0.33)mm、(0.48±0.20)mm;CAL术后3个月、6个月分别为(2.16±0.57)mm、(2.07±0.38)mm;RES评分为(8.20±1.34)。结论:冠向复位瓣用于治疗MillerⅠ类牙龈退缩能获得较高的根面覆盖率,长期效果有待进一步观察。

Objective:To observe the clinical effect of coronally advanced flap in the treatment of gingival recession of single tooth.Methods:Twelve patients with single tooth gingival recession were enrolled,and all of them were treated by coronally advanced flap.The probing depth(PD),gingival recession height(RGH)and clinical attachment level(CAL)were recorded at preoperation,3 months and 6 months postoperation.Root coverage rate(RC)was calculated at 3 and 6 months postoperation.A root coverage esthetic scoring system was used to record aesthetic scores.Results:There was no significant change in PD before and after operation.RGH were(0.63±0.33)mm and(0.48±0.20)mm at 3 and 6 months after operation.CAL were(2.16±0.57)and(2.07±0.38)mm at 3 and 6 months postopertion.The RES score was 8.20±1.34.Conclusion:Coronally advanced flap for the treatment of MillerⅠgingival recession can obtain prominent root coverage rate.However,the long-term effect needs to be observed.
