中药治疗旋后外旋及旋前外旋Ⅲ°~Ⅳ°踝关节骨折的疗效观察

Efficacy of traditional Chinese medicine on Ⅲ°~Ⅳ° ankle fractures of supination- external rotation and pronation-external rotation

目的探讨中药治疗旋后外旋及旋前外旋Ⅲ°~Ⅳ°踝关节骨折的疗效.方法选用86例经Lauge-Hansen分型判定为旋后外旋及旋前外旋Ⅲ°~Ⅳ°骨折患者,依照随机数字表法分为实施踝关节手术治疗的对照组(n=43例)及在对照组基础上辅以中药一盘珠汤治疗的观察组(n=43例),比较两组肿胀疼痛情况、关节功能恢复及并发症状况.结果两组治疗前肿胀与疼痛评分均无明显差异(P>0.05),经治疗后,两组肿胀与疼痛评分均有所下降,观察组低于对照组(P<0.05);两组治疗前Baird评分、AOFAS评分均无显著差异(P>0.05),经治疗后,两组Baird评分、AOFAS评分均有所上升,观察组Baird评分、AOFAS评分高于对照组(P<0.05),观察组肿胀消失时间、疼痛消失时间及愈合时间短于对照组(P<0.05);观察组并发症发生率为4.7%,相较于对照组的25.6%更低(P<0.05).结论旋后外旋及旋前外旋Ⅲ°~Ⅳ°踝关节骨折术后应用中药一盘珠汤加减治疗,疗效确切,术后肿胀疼痛改善、关节功能恢复加快及并发症情况降低.

Objective To explore the efficacy of traditional Chinese medicine onⅢ°~Ⅳ°ankle fractures of supinationexternal rotation and pronation-external rotation.Met hods Eighty-six patients withⅢ°~Ⅳ°fractures of supination-external rotation and pronation—external rotation determined by Lauge-Hansen classification were selected and divided into the control group(n=43,given ankle joint surgery)and the observation group(n=43,supplemented with traditional Chinese medicine Yipanzhu decoction on the basis of the control group)according to the random number table method.The swelling and pain,joint function recovery and complications were compared between the two groups.Resu I ts There were no significant differences in the scores of swelling and pain between the two groups before treatment(P>0.05).After treatment,the scores of swelling and pain in the two groups were decreased,and the scores in the observation group were lower than those in the control group(P<O.05).There were no significant differences in the Baird score and AOFAS score between the two groups before treatment(P>0.05).After treatment,the Baird score and AOFAS score of the two groups were increased,and the Baird score and AOFAS score of the observation group were higher than these of the control group(P<0.05).The swelling disappearance time,pain disappearance time and healing time in the observation group were shorter than those in the control group(P<0.05).The incidence rate of complications in the observation group was lower than that in the control group(4.7%vs.25.6%)(P<0.05).Cone I us i on After surgery ofⅢ°~Ⅳ°ankle fractures of supinationexternal rotation and pronation-external rotation,modified traditional Chinese medicine Yipanzhu decoction has an exact efficacy,and can improve the postoperative swelling and pain,quicken joint function recovery and reduce the complications.
