ICU机械通气患者应用集束化镇痛镇静护理模式的效果及对意外事件的影响

Effect of cluster analgesia sedation nursing mode on ICU patients with mechanical ventilation and its influence on accidents

目的探讨ICU机械通气患者应用集束化镇痛镇静护理模式的效果及对意外事件的影响。方法选取2017年10月至2018年10月在该院进行ICU机械通气的患者120例,按照完全随机法分为一般干预组和集束化干预组,各60例,并对两组患者进行分别护理。对两组患者丙泊酚、右美托咪定等麻醉药物用量以及机械通气时间、ICU住院时间进行统计,对两组患者干预前后心理状况、生活质量进行评价,统计两组患者意外事件发生情况以及家属满意情况。结果集束化干预组患者丙泊酚、右美托咪定用量均低于一般干预组,集束化干预组患者机械通气时间及ICU住院时间均短于一般干预组,差异有统计学意义(P<0.05)。干预后两组患者抑郁、焦虑评分低于干预前,生活质量评分高于干预前,且集束化干预组患者抑郁、焦虑评分低于一般干预组,生活质量评分高于一般干预组,差异有统计学意义(P<0.05)。集束化干预组患者意外事件发生率显著低于一般干预组,差异有统计学意义(P<0.05)。集束化干预组患者家属总满意率显著高于一般干预组,差异有统计学意义(P<0.05)。结论使用集束化镇痛镇静护理模式对ICU机械通气患者进行干预,能够减少镇静药物的用量,缩短患者机械通气时间、ICU住院时间,舒缓患者负面情绪,提升患者生活质量,减少意外事件的发生,使患者家属更加满意。

Objective To investigate the effect of cluster analgesia sedation nursing mode on ICU patients with mechanical ventilation and its influence on accidents.Methods A total of 120 patients who received mechanical ventilation in ICU in this hospital from October 2017 to October 2018 were randomly divided into general intervention group and cluster intervention group,with 60 patients in each group,and the two groups were respectively nursed.The dosage of propofol,dexmedetomidine and other anesthetic drugs,the duration of mechanical ventilation and the length of stay in ICU were counted in the two groups.The psychological status and quality of life of the two groups before and after intervention were evaluated.The incidence of accidents and the satisfaction of family members in the two groups were counted.Results The dosages of propofol and dexmedetomidine in the cluster intervention group were lower than those in the general intervention group,and the duration of mechanical ventilation and ICU stay in the cluster intervention group were shorter than those in the general intervention group,and the differences were statistically significant(P<0.05).After intervention,the scores of depression and anxiety in 2 groups were lower than before intervention,and the scores of quality of life in 2 groups were higher than before intervention,and the scores of depression and anxiety in the cluster intervention group were lower than the general intervention group,and the scores of quality of life in the cluster intervention group were higher than the general intervention group,with statistical significance(P<0.05).The incidence of accidents in the cluster intervention group was significantly lower than that in the general intervention group,and the difference was statistically significant(P<0.05).The total satisfaction rate of family members in the cluster intervention group was significantly higher than that in the general intervention group,and the difference was statistically significant(P<0.05).Conclusions Intervention with cluster analgesia sedation nursing mode on patients with mechanical ventilation in ICU can reduce the dosage of sedative drugs,shorten the duration of mechanical ventilation and ICU stay,relieve the negative emotions of patients,improve the quality of life of patients,reduce the occurrence of accidents,and make the patients'families more satisfied.
