摘要
目的探讨床旁超声胃窦单切面法评估胃残余量在机械通气患者肠内营养中的临床价值。方法将2019年6月至2020年3月本院重症监护肠内营养的机械通气患者100例随机分为2组,超声组用床旁超声胃窦单切面法而传统组用鼻饲注射器回抽法评估胃残余量。结果超声组肠内营养中断率低于对照组(P <0.05);达到目标喂养量时间、住ICU时间,操作时间较传统组缩短(P <0.05)。返流、误吸、消化道出血及呼吸机相关性肺炎发生率两组无明显差异(P> 0.05)。结论床旁超声胃窦单切面法评估机械通气患者胃残余量无创、便捷且准确,值得临床推广应用。
Objective To evaluate the application of gastric residual volume evaluated through gastric antral cross-sectional area by bedside ultrasound on enteral nutrition in mechanically ventilated patients. Methods A total of 100 mechanically ventilated patients with enteral nutrition in our hospital from June 2019 to March 2020 were randomly divided into two groups. In the ultrasound group, gastric residual volume was evaluated by bedside ultrasonic gastric antral cross-sectional area, while in the traditional group, gastric residual volume was evaluated by nasogastric syringe suction method. Results The interruption rate of enteral nutrition in ultrasound group was lower than that in control group(P < 0.05). The time to reach the target amount of feeding, stay in ICU and operation time are shorter than the traditional group(P < 0.05). There were no significant differences in the incidence of reflux, aspiration, gastrointestinal bleeding and ventilator-associated pneumonia(P > 0.05). Conclusion The method is noninvasive, convenient and accurate, and worthy of clinical application.
