糖尿病周围神经病变震动感觉阈值测定与分析 被引量:2

Determination and analysis of vibration sensory threshold in diabetic peripheral neuropathy

摘要 目的对比不同病程及不同年龄糖尿病患者震动感觉阈值(VPT)测定结果。方法选取2019年3月~2020年3月期间本院收治的糖尿病患者80例作为研究对象,按照年龄分为<50岁(n=20)、50~60岁(不含60岁)(n=20)、60~70岁(不含70岁)(n=20)、≥70岁(n=20),按照病程不同分为<1年(n=22)、1~5年(不含5年)(n=19)、5~10年(不含10年)(n=19)、10年(n=20),全部患者均给予患者震动感觉阈值测定,对比不同病程及不同年龄VPT,而且对比不同年龄段及不同病程VPT,而且对比不同年龄段及不同病程糖脂代谢水平、氧化应激水平。结果不同年龄段间左下肢及右下肢VPT对比具有明显差异(P<0.05),年龄越大,VPT越大,右下肢及左下肢对比无明显差异(P>0.05)。不同病程间左下肢及右下肢VPT对比具有明显差异(P <0.05),年龄越大,VPT越大,右下肢及左下肢对比无明显差异(P> 0.05)。不同年龄HDL、FPG、HbA1c、TC、TG、LDL、HDL水平对比具有明显差异(P <0.05)。不同病程HDL、FPG、HbA1c、TC、TG、LDL、HDL水平对比具有明显差异(P <0.05)。不同年龄SOD、GSH-PX、ROS水平对比具有明显差异(P<0.05)。不同病程SOD、GSH-PX、ROS水平对比具有明显差异(P<0.05)。通过直线相关分析各因素间的相关性,结果表明,左下肢及右下肢VPT与病程、FPG、HbA1c、SOD、GSH-PX、ROS间均呈显著正相关(P<0.05),与HDL、TC、TG、LDL、HDL均无明显相关性(P>0.05)。结论 VPT测定值与病程、血糖、年龄、氧化代谢指标有关。 Objective To compare the measurement results of vibration sensation threshold(VPT)in diabetic patients with different disease course and different ages. Methods Eighty patients with diabetes admitted to our hospital from March 2019 to March 2020 were selected as research objects. According to the age, they were divided into < 50 years group(n = 20), 50 ~ 60 years group(including 60 years)(n=20), 60~70 years group(including 70 years)(n=20), 70 years group(n=20). According to the course of disease, they were divided into < 1 years group(n = 22), 1 ~ 5 years group(including 5 years)(n = 19), 5 ~ 10 years group(including 10 years)(n = 19), 10 years group(n = 20). VPT of different disease course and different age were determined and compared, and glucose and lipid metabolism and oxidative stress levels of each group were compared. Results The VPT of left lower limb and right lower limb was significantly different in different age groups(P<0.05). The older the age was, the greater the VPT was, but there was no significant difference between right lower limb and left lower limb(P>0.05). The VPT of left lower limb and right lower limb was significantly different between different disease courses(P<0.05). The older the age was, the greater the VPT was, while the difference of right lower limb and left lower limb was not significant(P>0.05). The levels of HDL, FPG, Hb A1 c, TC, TG, LDL and HDL were significantly different in different ages(P<0.05).The levels of HDL, FPG, Hb A1 c, TC, TG, LDL and HDL were significantly different in different course of disease(P < 0.05). SOD,GSH-PX and ROS levels were significantly different at different ages(P<0.05). SOD, GSH-PX and ROS levels were significantly different in different course of disease(P < 0.05). The linear correlation analysis showed that VPT of left lower limb and right lower limb were significantly positively correlated with course of disease, FPG, Hb A1 c, SOD, gsh-px and ROS(P < 0.05), but were not significantly correlated with HDL, TC, TG, LDL and HDL(P>0.05). Conclusion VPT is related to disease course, blood glucose, age and oxidative metabolism.
