基于DRGs的新疆公立医院医疗服务绩效评价

Medical service performance evaluation of public hospitals in Xinjiang based on DRGs

目的评价2018年新疆21家公立医院的医疗服务绩效,并探讨相关工作的成效及存在的问题。方法通过收集新疆21家医院的病案首页信息,利用上海联众版的DRGs作为风险调整工具,同时运用加权秩和比法进行绩效评价分析。结果研究的21家样本医院病案首页数据总体入组率为99.55%,入组情况良好;在三级综合医院中医疗服务绩效排名第一的是A医院;在三级专科医院绩效评价中,排在前三位的分别是J医院、I医院和H医院;在二级医院绩效评价中T医院排名第一,优势较为明显。结论提升医院对DRGs的重视程度、强化病案首页的质量控制有助于提高DRGs绩效评价工作;基于DRGs的医疗服务绩效评价能够有效推进医院医疗卫生服务的均衡发展并引导医院明确自身功能定位,体现其公益性、促进医院正向发展。

Objective To evaluate the medical service performance of 21 public hospitals in Xinjiang in 2018, and to explore the effectiveness and problems of related work. Methods By collecting the first page information of medical records of 21 hospitals in Xinjiang, the DRGs of shanghai united edition was used as the risk adjustment tool, and the weighted rank sum ratio method was used to evaluate the performance. Results The overall enrollment rate of the front page data of 21 sample hospitals was 99.55%, which was in good condition;the hospital which ranked the first in the performance of TCM service in the third level general hospital;hospital J, hospital I and hospital h which ranked the first in the performance evaluation of the third level specialized hospital;Hospital t ranked the first in the performance evaluation of the second level hospital, with obvious advantages. Conclusions Enhancing the hospital’s attention to DRGs and strengthening the quality control of the front page of medical records are helpful to improve the performance evaluation of DRGs;the performance evaluation of medical services based on DRGs can effectively promote the balanced development of hospital medical and health services, guide the hospital to define its own functional orientation, reflect its public welfare and promote the positive development of the hospital.
