摘要
目的:通过18F-FDG PET/CT和MRI单独检查及两者联合检查的相关指标进行对比,探讨PET-MRI联合应用多模态数据对前列腺癌的诊断价值。方法:收集2019年1月—12月就诊于我院肿瘤科,临床诊断疑似前列腺癌并且已经完成了18F-FDG PET/CT、MRI以及病理学检查的患者58例,比较各种检查方法诊断的灵敏性、特异性、阳性预测值、阴性预测值、阳性似然比和阴性似然比。结果:共纳入患者58例,其中行病理性穿刺或手术确认患者30例,年龄介于64~88岁,平均年龄69.7岁。18F-FDG PET/CT阳性有33例,前列腺增生25例,灵敏度83.3%,特异度71.4%,阳性预测值75.8%,阴性预测值80.0%,阳性似然比2.913,阴性似然比0.234;MRI T2WI+DWI检查结果为Pca的患者为35例,前列腺其他良性疾病为23例,灵敏度93.3%,特异度75%,阳性预测值80%,阴性预测值91.3%,阳性似然比3.732,阴性似然比0.089;18F-FDG PET/CT和MRI联合诊断确诊Pca的有10例,前列腺其他病例为48例。灵敏度80.0%,特异度96.4%,阳性预测值96.0%,阴性预测值81.8%,阳性似然比22.222,阴性似然比0.207。结论:PET-MRI多模态诊断对于前列腺癌的诊断具有更高的预测价值。
Objective To investigate the value of multimodal PET-MRI data in the diagnosis of prostate cancer by comparing the relative indexes of 18F-FDG PET/CT and MRI alone examination and combined examination.Methods Collected in January 1,2019—December 31,2019 in the department of oncology and clinical diagnosis of suspected of prostate cancer and had perfect of 18F-FDG PET/CT,MRI and pathology of patients,comparison of various examination methods in the diagnosis of sensitivity,specificity,positive predictive value,negative predictive value,positive likelihood ratio and negative likelihood ratio.Results A total of 58 patients were included,of which 30 were confirmed by pathologic puncture or surgery,ranging in age from 64 to 88 years old with an average age of 69.7 years old.There were 33 positive cases of 18F-FDG PET/CT,25 positive cases of prostatic hyperplasia,sensitivity 83.3%,specificity 71.4%,positive predictive value 75.8%,negative predictive value 80.0%,positive likelihood ratio 2.913,negative likelihood ratio 0.234;MRI T2WI+DWI showed PCA in 35 patients and other benign prostate diseases in 23 patients,with sensitivity of 93.3%,specificity of 75%,positive predictive value of 80%,negative predictive value of 91.3%,positive likelihood ratio of 3.732,negative likelihood ratio of 0.089.The combined diagnosis of 18F-FDG PET/CT and MRI confirmed PCA in 10 cases and other prostate in 48 cases.The sensitivity was 80.0%,the specificity was 96.4%,the positive predictive value was 96.0%,the negative predictive value was 81.8%,the positive likelihood ratio was 22.222,the negative likelihood ratio was 0.207.Conclusion The multimodal diagnosis of PET-MRI has higher predictive value for the diagnosis of prostate cancer.
