降脂通脉汤对冠心病心绞痛患者血脂代谢及血管内皮功能的影响

The Effect of Jiangzhi Tongmai Decoction on Blood Lipid Metabolism and Vascular Endothelial Function in Patients with Angina Pectoris of Coronary Heart Disease

目的探究降脂通脉汤对冠心病心绞痛患者血脂代谢及血管内皮功能的影响。方法选取中国医科大学附属第一医院2018年12月—2019年12月心内科收治的冠心病心绞痛患者72例,根据随机数字表法分为对照组和试验组,各36例,对照组患者给予单纯西药治疗方案,试验组在对照组的基础上予以降脂通脉汤。治疗结束后对比2组临床疗效;采用视觉疼痛模拟量表(Visual analogue scale,VAS)对2组心绞痛发作时疼痛程度进行评价;比较2组血清总胆固醇(Total cholesterol,TC)、三酰甘油(Triglycerides,TG)、高密度脂蛋白胆固醇(High density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(Low-density lipoprotein cholesterol,LDL-C)水平以观察血脂水平;比较2组血清血管内皮生长因子(Vascular endothelial growth factor,VEGF)及内皮素(Endothelin,ET)水平以观察血管内皮功能;记录2组不良反应发生情况。结果治疗后,试验组总有效率为91.67%(33/36),对照组总有效率为69.44%(25/36),试验组高于对照组,差异有统计学意义(P<0.05);治疗后与对照组比较,试验组患者发作持续时间、每周发作频率及VAS评分较低(P<0.05);与对照组治疗后比较,试验组患者TC、TG、LDL-C水平较低,HDL-C水平较高(P<0.05);与对照组治疗后比较,试验组患者VEGF、NO及ET水平较高(P<0.05)。结论降脂通脉汤治疗冠心病心绞痛可能通过调节血脂代谢,改善血管内皮功能,从而发挥显著的治疗效果。

Objective To explore the effect of Jiangzhi Tongmai decoction on blood lipid metabolism and vascular endothelial function in patients with angina pectoris.Methods 72 patients with coronary heart disease and angina pectoris admitted to the First Affiliated Hospital of China Medical University from December 2018 to December 2019 were divided into control group and study group according to the randomized control table,including 36 cases in the study group and 36 cases in the control group.The patients in the control group received simple western medicine treatment plan,and the study group received more with Jiangzhi Tongmai decoction.After treatment,the clinical efficacy of the two groups was compared.The Visual analogue scale(VAS)was used to evaluate the degree of pain during angina attacks in the two groups.Serum total cholesterol(TC),triglycerides(TG)and high density lipoprotein(HDL)were compared between the two groups,cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C)levels were measured to determine the level of blood lipid.The serum levels of vascular endothelial growth factor(VEGF)and endothelin(ET)were compared between the two groups.The endothelial function was observed.The incidence of adverse reactions in the two groups was recorded.Results After treatment,the total effective rate of the experimental group was 91.67%(33/36),and the total effective rate of the control group was 69.44%(25/36),the experimental group was higher than the control group,the difference was statistically significant(P<0.05).After treatment,compared with the control group,the duration of attack,weekly frequency of attack and VAS score in experimental group were lower(P<0.05);Compared with the control group after treatment,the levels of TC,TG and LDL-C in experimental group were lower,and the level of HDL-C was higher(P<0.05);Compared with the control group after treatment,VEGF,NO and ET levels in experimental group were higher(P<0.05).Conclusion Jiangzhi Tongmai decoction can improve vascular endothelial function by regulating blood lipid metabolism,so as to play a significant therapeutic effect.
