摘要
目的比较无创呼吸机与鼻导管吸氧对慢性阻塞性肺疾病急性加重呼吸衰竭患者疗效及动脉血气指标影响。方法选取安徽省宿州市第一人民医院2016年12月—2020年6月收治的90例慢性阻塞性肺疾病急性加重呼吸衰竭患者,按随机数字表法分为两组:鼻导管吸氧组(45例)接受鼻导管持续低流量吸氧;无创呼吸机组(45例)接受无创呼吸机吸氧。观察两组患者治疗相关指标、动脉血气指标和肺功能指标、生活质量评分及咽部不适发生情况。结果无创呼吸机组住院时间、ICU住院时间、再发急性加重率低于鼻导管吸氧组(P<0.05)。治疗前组间血气指标无显著差异(P>0.05),治疗后两组血气指标均显著改善(P<0.05),无创呼吸机组FEV1、FEV1/FVC、FEV1%预测值高于鼻导管吸氧组(P<0.05),无创呼吸机组PaO2显著高于鼻导管吸氧组(P<0.05),PaCO2显著低于鼻导管吸氧组(P<0.05)。治疗前组内WHOQOL-BREF评分差异无统计学意义(P>0.05),无创呼吸机组治疗后30 d、90 d WHOQOL-BREF评分均高于鼻导管吸氧组(P<0.05)。两组咽部不适发生率差异不大(鼻导管及氧组8.9%,无创呼吸机组11.1%,P>0.05)。结论无创呼吸机在慢性阻塞性肺疾病急性加重呼吸衰竭患者中疗效显著,可改善肺功能、动脉血气指标及生活质量。
Objective To compare the effects of noninvasive ventilator and nasal cannula oxygen inhalation on patients with acute exacerbation of respiratory failure in chronic obstructive pulmonary disease.Methods 90 patients with acute exacerbation of chronic obstructive pulmonary disease who were admitted to the First People's Hospital of Suzhou City from December 2016 to June 2020 were selected and divided into two groups according to the random number table meth‐od:the nasal catheter inhalation group(45 cases)received continuous low-flow oxygen inhalation by nasal cannula.The non-invasive breathing unit group(45 cases)received non-invasive ventilator for oxygen inhalation.The changes of treat‐ment-related indexes,arterial blood gas indexes and lung function indexes,quality of life scores and throat discomfort in the two groups of patients were observed.Results The hospitalization time,ICU hospitalization time,and relapse acute exacerbation rate of the non-invasive breathing unit group were lower than those of the nasal catheter inhalation group(P<0.05).There was no significant difference in blood gas indexes between groups before treatment(P>0.05).After treat‐ment,the blood gas indexes of both groups were significantly improved(P<0.05),and FEV1,FEV1/FVC,FEV1%in the non-invasive breathing unit group were higher than that of the nasal catheter inhalation group(P<0.05),PaO2 in the non invasive breathing unit group was significantly higher than that of the nasal catheter inhalation group(P<0.05),and Pa‐CO2 was significantly lower than that of the nasal catheter inhalation group(P<0.05).There was no statistically signifi‐cant difference in the WHOQOL-BREF score before treatment(P>0.05).The 30-day and 90-day WHOQOL-BREF scores of the non-invasive breathing unit group were higher than those of the nasal catheter inhalation group(P<0.05).There was no significant difference in the incidence of throat discomfort between the two groups(8.9%vs 11.1%,P>0.05).Conclusion Non-invasive ventilator is effective in patients with acute exacerbation of chronic obstructive pulmo‐nary disease and can improve lung function,arterial blood gas index and quality of life.
