半髋关节置换术治疗老年不稳定股骨转子间骨折的效果观察

Effect observation of hemiarthroplasty in the treatment of unstable intertrochanteric fracture of femur in the elderly

目的研究半髋关节置换术治疗老年不稳定股骨转子间骨折的效果以及相关指标。方法60例老年不稳定股骨转子间骨折患者,依据不同手术方式分为实验组与对照组,每组30例。实验组采取半髋关节置换术治疗,对照组采取髓内钉固定术治疗。比较两组手术相关指标、并发症发生情况、术后髋关节功能。结果实验组手术时间(65.93±12.16)min、下地时间(7.48±1.14)d短于对照组的(74.26±18.57)min、(8.52±2.47)d,出血量(334.49±70.52)ml少于对照组的(388.37±79.21)ml,差异均有统计学意义(P<0.05)。实验组并发症发生率3.33%低于对照组的26.67%,差异有统计学意义(P<0.05)。实验组总优良率96.67%高于对照组的76.67%,差异有统计学意义(P<0.05)。结论老年不稳定股骨转子间骨折患者应用半髋关节置换术治疗效果较好,可明显缩短手术时间,减少术中出血量,降低术后并发症的发生几率,更利于患者术后髋关节功能的恢复,值得在临床进一步推广实施。

Objective To study the effect and related indexes of hemiarthroplasty in the treatment of unstable intertrochanteric fracture of femur in the elderly.Methods A total of 60 elderly patients with unstable intertrochanteric fracture of femur were divided into experimental group and control group according to different surgical methods,with 30 cases in each group.The experimental group was treated with hemiarthroplasty,and the control group was treated with intramedullary nail fixation.The surgical related indexes,occurrence of complications,and postoperative hip function were compared between the two groups.Results The operation time(65.93±12.16)d and off-bed time(7.48±1.14)min of the experimental group were shorter than(74.26±18.57)min and(8.52±2.47)min of the control group,and blood loss(334.49±70.52)ml was less than(388.37±79.21)ml of the control group.All the difference was statistically significant(P<0.05).The incidence of complications 3.33%of the experimental group was lower than 26.67%of the control group,and the difference was statistically significant(P<0.05).The total excellent rate 96.67%of the experimental group was higher than 76.67%of the control group,and the difference was statistically significant(P<0.05).Conclusion Hemiarthroplasty has good therapeutic effect on elderly patients with unstable intertrochanteric fracture of femur,which can obviously shorten the operation time,reduce intraoperative blood loss,lower the incidence of postoperative complications,and is more conducive to the recovery of hip joint function after operation.It is worthy of further promotion in clinical practice.
